CPT code 63046 – Thoracic laminectomy, facetectomy and foraminotomy

Code Definition

63046 is the CPT code for a laminectomy, facetectomy and foraminotomy at a single thoracic vertebral segment. The surgeon decompresses the spinal cord or nerve roots, unilaterally or bilaterally, most often for spinal or lateral recess stenosis.

Each additional vertebral segment is reported with add-on code 63048. A thoracic laminectomy without facetectomy or foraminotomy is a different code, 63003, and the same procedure at a lumbar segment is 63047.

Key takeaways

Key takeaways

CPT code 63046 reports a laminectomy, facetectomy and foraminotomy at a single thoracic vertebral segment, performed unilaterally or bilaterally.

Facetectomy and foraminotomy are part of the 63046 descriptor. A thoracic laminectomy without them is reported with 63003 or 63016 instead.

The spinal region decides the code: 63045 is cervical, 63046 is thoracic and 63047 is lumbar. None of the three is an add-on code.

Each additional vertebral segment is reported with add-on code 63048. NCCI never allows the operating microscope code 69990 with 63046.

Pabau, the practice management platform we build, turns each invoice into a claim and submits it to US payers through Claim.MD.

CPT code 63046: Full descriptor and what it covers

CPT code 63046 is the primary code for a posterior laminectomy, facetectomy and foraminotomy at one thoracic vertebral segment. The American Medical Association (AMA) descriptor breaks down into four parts.

  • Procedure: Laminectomy, facetectomy and foraminotomy.
  • Side: Unilateral or bilateral.
  • Purpose: Decompression of the spinal cord, cauda equina and/or nerve root(s), for example in spinal or lateral recess stenosis.
  • Extent: A single vertebral segment in the thoracic spine.

First, the surgeon removes the lamina, resects part of the facet joint and enlarges the neural foramen at the same segment. Those three steps form one service, valued together under 63046, so the note has to document all three.

Two effects follow for coders. First, a thoracic laminectomy that leaves the facets and foramina untouched does not qualify for 63046. And because the descriptor already says unilateral or bilateral, a two-sided decompression is still one unit of 63046.

CPT 63046 vs 63045 and 63047: Choosing the spinal region

The 63045 to 63047 codes share one descriptor and differ only by spinal region. CPT 63045 is cervical, 63046 is thoracic, and CPT 63047 is lumbar. Also, each is a primary code that reports the first vertebral segment in its region.

CodeSpinal regionTypeWhen to use
63045CervicalPrimaryFirst cervical segment decompressed with laminectomy, facetectomy and foraminotomy
63046ThoracicPrimaryFirst thoracic segment decompressed with laminectomy, facetectomy and foraminotomy
63047LumbarPrimaryFirst lumbar segment decompressed with laminectomy, facetectomy and foraminotomy
63048Any regionAdd-onEach additional vertebral segment, listed after the primary code

First, read the region from the operative note, never from the diagnosis code alone. A decompression at T9 maps to 63046, while the same procedure at L4 maps to 63047. If the note names no vertebra, query the surgeon before the claim goes out.

NCCI pairs 63045 with 63046, and 63046 with 63047, with a modifier indicator of 1. However, at contiguous levels, such as T12 and L1, report 63046 plus 63048. Use two primaries with 59 or XS only for non-contiguous levels.

When two primary codes do apply, the modifier goes on the column-two code. In addition, the operative note must show the separate, non-contiguous segments that justify it.

CPT 63046 vs 63003, 63016 and 63055: Matching the code to the technique

Several thoracic decompression codes sit close to 63046, and each has its own entry in our searchable CPT code library. In fact, the operative technique is what separates them. The most common mix-up is with 63003, the thoracic laminectomy that does not include facetectomy or foraminotomy.

CodeTechniqueSegments covered
63046Laminectomy, facetectomy and foraminotomy, thoracicSingle segment; add 63048 per extra segment
63003Laminectomy without facetectomy, foraminotomy or discectomy, thoracic1 or 2 segments
63016Laminectomy without facetectomy, foraminotomy or discectomy, thoracicMore than 2 segments
63055Transpedicular decompression of cord or nerve roots, thoracic (eg, herniated disc)Single segment; add-on 63057 per extra segment
63266Laminectomy for excision or evacuation of an extradural intraspinal lesion other than neoplasm, thoracicPer procedure

NCCI bars 63003 and 63055 when billed with 63046, so only one of each pair is paid for the same segment. Also, the intraspinal lesion codes, such as 63266, are broader and include the decompression. Therefore, choose the code that matches the primary technique the operative report describes.

Thoracic disc herniation has no laminotomy code like the cervical 63020 or the lumbar 63030. Instead, a thoracic discectomy usually runs through the transpedicular or costovertebral codes instead, starting at 63055.

Add-on code 63048: Billing additional thoracic segments

CPT 63048 is the add-on code for each additional vertebral segment decompressed with laminectomy, facetectomy and foraminotomy. Also, it is reported with 63045, 63046 or 63047 and is never billed alone. List the primary code first, then one unit of 63048 per extra segment.

  • One thoracic segment (T10): Bill 63046 once, with no 63048.
  • Two segments (T10 and T11): Bill 63046 once plus 63048 once.
  • Three segments (T9 to T11): Bill 63046 once plus 63048 twice.

In addition, each unit of 63048 needs its own named segment in the operative report. Billing two units against a note that says “multilevel decompression” invites an audit. Modifier 51 is never appended to 63048, because add-on codes are exempt from the multiple procedure reduction.

CPT 63052 and 63053 are not options here. Those add-ons cover lumbar decompression during posterior interbody arthrodesis, so they never pair with a thoracic 63046. Finally, check the full code set on the surgical superbill against these rules before submission.

The path below combines the technique, region and segment rules into the three questions a coder answers from the note.

Decision path for CPT 63046
T12 and L1 count as contiguous levels, so that pair takes 63046 plus 63048 rather than a second primary code. Rules from the AMA CPT descriptors and the CMS NCCI Policy Manual.

Pro Tip

Ask surgeons to list every decompressed vertebra by name in a fixed field of the operative note template. Coders can then count 63048 units straight from the note, without a query.

What the surgeon does under CPT code 63046

Knowing the operative steps helps coders confirm that the note supports all three components of the code. The sequence below reflects a usual open posterior approach.

  1. Positioning and level check: The patient lies prone under general anesthesia. The surgeon confirms the thoracic level, usually with imaging, because thoracic levels are easy to miscount.
  2. Exposure: Next, a midline incision exposes the spinous processes and laminae of the target segment.
  3. Laminectomy: Then the surgeon removes the lamina to open the spinal canal and relieve pressure on the cord.
  4. Facetectomy: After that, part of the facet joint is resected to decompress the lateral recess.
  5. Foraminotomy: Next, the neural foramen is enlarged to free the exiting nerve root.
  6. Closure: Finally, the surgeon confirms the decompression, achieves hemostasis and closes in layers.

Fluoroscopy used to find the level is bundled into the procedure under NCCI. Therefore, it is not reported separately with 63046.

Documentation requirements for CPT code 63046

The operative report is the main evidence for 63046, and auditors read it against the descriptor. Each element below maps to a common denial reason, so confirm all of them before the claim is coded.

Required elementWhat the operative report must state
Vertebral segmentName each thoracic segment decompressed, such as T10. “Thoracic spine” alone is not enough.
Segment countState how many segments were decompressed. Each one beyond the first supports a unit of 63048.
LaminectomyDescribe removal of the lamina at each named segment.
FacetectomyDescribe the facet resection at each segment. Without it, the procedure points to 63003 or 63016.
ForaminotomyDescribe the foraminal decompression and the nerve root freed.
Neural decompressionConfirm the spinal cord or nerve roots were decompressed and how that was verified.
Medical necessityLink the procedure to the diagnosis, the imaging findings and the clinical course.

Also, document the conservative care history and neurologic findings before surgery, not after a denial.

NCCI edits and codes billed with CPT 63046

The CMS NCCI procedure-to-procedure edits decide which codes pay alongside 63046. An indicator of 0 means no modifier can override the edit. In contrast, an indicator of 1 allows modifier 59 or an X modifier when the documentation supports it.

Edits in the table come from the October 2026 practitioner file, version 32.3. In addition, Medicare limits 69990 to a short list of procedures, and 63046 is not on it.

Thoracic arthrodesis, such as posterior fusion code 22610 and its add-on 22614, is a separate service. Report 63046 with thoracic arthrodesis only when the decompression is a distinct service at a documented segment, not just exposure for the fusion. Then check the current PTP file for the pair. Also, instrumentation add-ons such as 22840 and 22842 are reported with the arthrodesis code.

Modifiers for CPT code 63046

The Medicare fee schedule indicators for 63046 settle most modifier questions. The table below lists the modifiers that come up on thoracic decompression claims.

ModifierPurposeHow it applies to 63046
22Increased procedural servicesUse only when the work substantially exceeds the typical case, with the reason documented in the operative note
50Bilateral procedureDo not apply. The descriptor already covers unilateral or bilateral decompression (bilateral indicator 2)
51Multiple proceduresStandard reduction applies when 63046 is billed with another primary procedure. Never append it to 63048
59 or XSDistinct procedural service or separate structureOverrides an indicator-1 NCCI edit when the segments are distinct and documented
62Two surgeonsCo-surgery is permitted (co-surgeon indicator 2). Each surgeon documents their own portion
66Surgical teamNot permitted for 63046 (team surgery indicator 0)
80 or 82Assistant surgeonPayable for 63046 (assistant indicator 2). Use 82 when no qualified resident was available
24 or 79Unrelated service in the global periodRequired for an unrelated visit (24) or procedure (79) within the 90-day global period

Medicare reimbursement for CPT code 63046 (2026)

Medicare pays 63046 under the Physician Fee Schedule in facility settings only, because no non-facility value is set. The national figures below come from the CMS October 2026 relative value file. Check your locality rate in the CMS Physician Fee Schedule lookup tool.

ComponentCPT 63046CPT 63048 (add-on)
Work RVU16.823.38

National payment, qualifying APM participant ($33.5675)$1,190.30$188.31
Global period090ZZZ (follows the primary code)

Each payment equals total RVUs multiplied by the 2026 conversion factor, before locality adjustment. However, CY 2026 has two conversion factors, depending on whether the clinician is a qualifying APM participant.

The 90-day global period bundles routine postoperative visits into the 63046 payment. Bill an unrelated visit within that window only with modifier 24, and an unrelated procedure with modifier 79.

Prior authorization and payer criteria for CPT code 63046

Prior authorization rules for 63046 depend on the plan, so confirm them during insurance eligibility verification before scheduling. Also, commercial spine policies usually ask for imaging and a documented clinical course.

Payer typeReview requirementCriteria to document
Traditional MedicareNo prior authorization program covers 63046. It is not on the WISeR model list (July 2026)Medical necessity in the record, which a MAC can review after payment
Medicare AdvantageVaries by planFollow the plan’s own spine surgery policy
Commercial plans using Evolent spine criteriaFor example, BlueCross BlueShield of South Carolina lists 63046 and 63048 in its thoracic spine surgery policyAny one of three: progressive myelopathy; a progressive neurologic deficit with MRI or CT cord compression; or failed conservative care plus imaging. The third needs 6 consecutive weeks of care and compression at the matching level

Under that commercial policy, conservative care means at least two modalities over 6 consecutive weeks. For example, they include medication, physical therapy, a home exercise program or epidural injections. Record the dates and modalities before submitting the request.

ICD-10-CM diagnosis codes that support CPT 63046

CPT 63046 needs a thoracic diagnosis that matches the decompressed level. Otherwise, a lumbar or cervical diagnosis on a 63046 claim is a clear mismatch for payer edits.

ICD-10-CM codeDescriptionTypical use with 63046
M48.04Spinal stenosis, thoracic regionCanal or lateral recess stenosis as the primary indication
M47.14Other spondylosis with myelopathy, thoracic regionDegenerative compression of the cord with myelopathic signs
M51.04Intervertebral disc disorders with myelopathy, thoracic regionDisc disease compressing the cord
M51.24Other intervertebral disc displacement, thoracic regionDisplaced disc contributing to compression
M54.14Radiculopathy, thoracic regionNerve root symptoms that support the foraminotomy
M48.8X4Other specified spondylopathies, thoracic regionOssification of the posterior longitudinal ligament or ligamentum flavum

Also, code to the highest detail the documentation supports. Myelopathy codes carry more weight with payers than stenosis alone, so make sure the neurologic exam is in the record.

Common denial reasons for CPT code 63046

Most 63046 denials trace back to the region, the technique or the segment count. Tracking them by reason code, as part of denial management in healthcare, shows which of these causes repeats in your practice.

  • Wrong region: A lumbar decompression is billed as 63046 instead of 63047. Prevention: confirm the named vertebra before coding.
  • Missing facetectomy or foraminotomy: The note describes a laminectomy only. Prevention: report 63003 or 63016 instead, or query the surgeon.
  • Add-on units that do not match: The 63048 units exceed the segments named in the note. Prevention: count named segments, not levels mentioned in passing.
  • Bundled codes billed: 69990 or 62320 to 62327 are billed with 63046. Prevention: run every pair through the NCCI table above.
  • Modifier errors: Modifier 50 is added to 63046, or 51 to 63048. Prevention: follow the fee schedule indicators.
  • Global period conflicts: A postoperative visit is billed without modifier 24 or 79. Prevention: flag the 90-day window in the patient record.

How Pabau keeps thoracic decompression claims accurate

Spine claims fail when the operative note and the claim tell different stories. Pabau’s claims management builds each claim from the invoice already in Pabau, so your billing team does not re-key the claim lines.

Claims then go to US payers through Pabau’s Claim.MD clearinghouse connection. Real-time eligibility checks run before the surgery date, and claim status and remittances come back into the same system.

Pabau claims screen showing a claim built from the invoice, ready for submission
Pabau’s claims screen builds the claim from the invoice. The lines already entered there, such as 63046 and 63048, reach submission without re-keying.

In Pabau, custom operative note templates can make the segment list and the facetectomy and foraminotomy details required fields. The coder then works from a complete note, and fewer 63046 claims need a surgeon query.

Submit spine surgery claims without re-keying

Pabau builds each claim from the invoice and submits it through Claim.MD, so the 63046 and 63048 lines reach the payer as billed. Eligibility checks and claim tracking sit in the same system.

Conclusion

CPT code 63046 is simple to assign once the operative note answers two questions. Which thoracic segments were decompressed, and were the facets and foramina treated as well as the lamina? Each answer moves the claim between 63046, 63048, 63003 and 63047.

Watch the thoracolumbar junction most closely. For example, contiguous levels such as T12 and L1 take 63048, and a second primary code there invites a denial.

Finally, put your coding effort into the operative note before the claim exists. A template that forces the segment list and the three techniques heads off most denial causes before any appeal is needed.

Frequently asked questions

What does CPT code 63046 cover?

CPT code 63046 covers a laminectomy, facetectomy and foraminotomy at a single thoracic vertebral segment, unilateral or bilateral. It includes decompression of the spinal cord or nerve roots, most often for spinal or lateral recess stenosis.

What modifiers are used with CPT code 63046?

Common modifiers are 22 for substantially increased work and 59 or XS to override an indicator-1 NCCI edit. Modifier 62 covers co-surgeons, and 80 or 82 an assistant surgeon. Modifier 50 does not apply, because the descriptor already covers bilateral decompression.

Why is CPT code 63046 denied by payers?

Common causes are a lumbar procedure billed as 63046 and a note that omits the facetectomy or foraminotomy. Others are 63048 units that exceed the named segments, bundled codes such as 69990, and global period visits billed without modifier 24.

Can CPT 63046 and 63047 be billed together?

Only at non-contiguous thoracic and lumbar segments, with 59 or XS on 63047. Contiguous segments, such as T12 and L1, are 63046 plus 63048.

Is CPT 63046 an add-on code?

No. CPT 63046 is a primary code with a 90-day global period, and so are 63045 and 63047. The only add-on in this family is 63048, which reports each additional vertebral segment.
