HCPCS code V2319 – Trifocal seg width over 28 mm

Code Definition

V2319 is the HCPCS Level II code for a trifocal lens with a segment width over 28 mm. It reports a wide trifocal segment on its own line, next to the base trifocal lens code.

V2319 never replaces that base code and carries no power range. V2300 to V2314 describe the lens by power, while V2319 only reports the segment. Medicare pays for these lenses only under its eyeglass benefit for aphakia and for patients after cataract surgery with an intraocular lens.

Key takeaways

Key takeaways

HCPCS code V2319 reports a trifocal lens whose segment measures more than 28 mm wide, from one side to the other.

It is an add-on line. Bill it beside the base trifocal lens code from V2300 to V2314 or V2399, never in place of it.

V2319 carries no power range. The base code describes the sphere and cylinder, and V2319 only reports the wide segment.

Medicare excludes routine eyeglasses. It covers lenses for aphakia, and one pair after each cataract surgery with an intraocular lens implant.

Every V2319 line needs RT or LT. Bill a pair as two lines of one unit each, not one RTLT line with two units.

In Pabau, our practice management software, the prescription and the claim sit on one patient record, and claims are submitted and tracked from there.

HCPCS code V2319: Official descriptor and code definition

HCPCS code V2319 is the Level II code for a trifocal lens with a segment width over 28 mm. The official descriptor, published by the Centers for Medicare and Medicaid Services (CMS), reads “Trifocal seg width over 28 mm”. The short descriptor is “Lens trifocal seg width > 28”.

The segment, or seg, is the reading window set into the lower part of a multifocal lens. In a trifocal it holds both the intermediate and the near zones. Seg width is how wide that window measures from side to side, in millimeters.

Standard flat-top trifocal segments are 25 mm or 28 mm wide, and neither one qualifies. A 35 mm segment, or a full-width Executive-style segment, is over the threshold and takes a V2319 line.

Where V2319 fits in the trifocal lens range (V2300 to V2399)

The trifocal block holds two kinds of code. Base lens codes describe the lens by its sphere and cylinder power, and each lens takes exactly one. Add-on codes describe a feature of that lens and go on extra lines. The Refractive Lenses policy article A52499 lists V2319 among the add-ons.

Code CMS descriptor Role on the claim
V2300-V2302 Sphere, trifocal, split into three sphere power bands, per lens Base lens
V2303-V2313 Spherocylinder, trifocal, split by sphere band and cylinder band, per lens Base lens
V2314 Spherocylinder, trifocal, sphere over plus or minus 12.00d, per lens Base lens
V2315 Lenticular, (myodisc), per lens, trifocal Add-on feature
V2318 Aniseikonic lens, trifocal Add-on feature
V2319 Trifocal seg width over 28 mm Add-on feature (this code)
V2320 Trifocal add over 3.25d Add-on feature
V2321 Lenticular lens, per lens, trifocal Add-on feature
V2399 Specialty trifocal (by report) Base lens, by report

Read the table by role, not by number. A trifocal claim always starts with one base code per lens. V2319 joins it only when the segment on that lens is wider than 28 mm. Swapping the base code for V2319 leaves the lens itself unbilled.

A wide segment can sit on a lens that also has a strong add. In that case the lens carries three lines: the base code, V2319 and V2320, each with its own RT or LT.

The diagram below runs that decision for one lens at a time and shows how the lines stack on a claim for a pair.

Decision diagram and claim-line example for HCPCS V2319
Each eye gets its own base line and its own V2319 line, so a pair with wide segments on both lenses bills four lines. Descriptors follow CMS HCPCS and policy article A52499.

V2319 is not a lens power code

Some code lookups and older guides attach a sphere and cylinder range to V2319. That range belongs to V2107, a single-vision spherocylinder code. The single-vision spherocylinder codes run from V2103 to V2114.

V2319 says nothing about power. The base code carries the prescription, and V2319 only reports the segment. The bifocal block has its own version of this code. V2219 reports a bifocal seg width over 28 mm, so the lens design decides which segment code applies.

Medicare coverage for V2319: the eyeglass exclusion and its exception

Medicare Part B does not pay for routine eyeglasses. Section 1862(a)(7) of the Social Security Act excludes them, along with eye exams for prescribing glasses. A wide-segment trifocal for an ordinary presbyopia prescription is therefore not covered.

The exception sits in the prosthetic device benefit, Section 1861(s)(8). It covers one pair of conventional eyeglasses or contact lenses after each cataract surgery with insertion of an intraocular lens. Patients with aphakia, including congenital aphakia, also qualify for corrective lenses.

The pseudophakic benefit attaches to each surgery, not to the patient. A later operation on the second eye supports a second pair. If both eyes are operated on before any glasses are supplied, only one pair is covered after the second surgery.

Within that benefit, V2319 is billed alongside the base lens. The four DME Medicare Administrative Contractors apply LCD L33793 and its policy article. Payment comes from the DMEPOS fee schedule, so check the current amount for your state before you quote a patient.

  • Covered: A trifocal segment over 28 mm on the pair supplied after a qualifying cataract surgery with an IOL, or on lenses for aphakia.
  • Statutorily excluded: Routine glasses for a patient with no aphakia and no IOL. The segment width does not change that.
  • Replacements: Replacement lenses for pseudophakic patients are noncovered. For aphakic patients, replacements are covered when medically necessary.
  • Upgrades: A progressive lens is billed as the standard trifocal codes plus V2781 for the price difference. The policy denies that difference as noncovered.
  • Other payers: Medicare Advantage and commercial vision plans set their own lens rules, so verify the benefit before you order.

Only suppliers enrolled with Medicare to furnish DMEPOS can bill these lens codes to a DME MAC. A practice that dispenses without that enrollment cannot send V2319 to Medicare.

ICD-10-CM codes that support a V2319 claim

The Refractive Lenses policy accepts six diagnosis codes for refractive lenses. A Medicare claim needs one of them on each lens line, including the V2319 line.

ICD-10-CM code Description When it applies
Z96.1 Presence of intraocular lens Pseudophakia after cataract surgery with an IOL
H27.00 Aphakia, unspecified eye Aphakia where the record does not name the eye
H27.01 Aphakia, right eye Aphakia in the right eye
H27.02 Aphakia, left eye Aphakia in the left eye
H27.03 Aphakia, bilateral Aphakia in both eyes
Q12.3 Congenital aphakia Lens absent from birth

Match the eye in an aphakia code to the RT or LT modifier on the lens line. Z96.1 carries no laterality, so the operative note has to show which eye received the implant.

A refractive diagnosis such as H52.4 (presbyopia) explains why the patient needs a trifocal. It does not establish Medicare coverage on its own. Commercial vision plans set their own diagnosis rules, so check the plan policy.

Modifiers for HCPCS code V2319

The Refractive Lenses policy requires RT or LT on every lens code it covers, V2319 included. Lines without them are rejected as incorrect coding.

Modifier Meaning When to use it on V2319
RT Right side The wide segment is on the right lens
LT Left side The wide segment is on the left lens
GA Waiver of liability statement issued as required by payer policy Coverage criteria are not met and the patient signed an advance beneficiary notice (ABN)
GZ Item or service expected to be denied as not reasonable and necessary Coverage criteria are not met and no ABN is on file
GY Item or service statutorily excluded Routine glasses outside the eyeglass benefit, when a secondary payer needs a Medicare denial

When both lenses have the wide segment, bill two V2319 lines, one RT and one LT, with one unit each. A single line with RTLT and two units is rejected. That rule has applied to dates of service from March 1, 2019.

The policy ties the KX modifier to anti-reflective coating, tints, oversized lenses and polycarbonate lenses. V2319 is not on that list. A routine V2319 line needs laterality, plus GA, GZ or GY only when coverage fails.

How to bill HCPCS code V2319 step by step

A clean V2319 claim starts at the dispensing table, with the lens order in front of you. Work through these steps for each lens.

  1. Confirm the coverage path. For Medicare, look for Z96.1 with a recent cataract surgery, or an aphakia diagnosis. Without one, the pair is statutorily excluded.
  2. Check the segment width for each eye. Only a segment wider than 28 mm meets the descriptor. A 28 mm flat-top takes no V2319 line.
  3. Code the base lens first. Choose the V2300 to V2314 code that matches the sphere and cylinder, or V2399 for a specialty trifocal.
  4. Add the V2319 line. Put it on its own line beside the base lens, with RT or LT and one unit.
  5. Add other features only when ordered. An add over +3.25D (V2320) or an aniseikonic lens (V2318) also takes its own line.
  6. Point each line to the diagnosis. Link every lens line to Z96.1, an H27.0- aphakia code, or Q12.3.
  7. Apply liability modifiers if needed. Use GA with a signed ABN, GZ without one, or GY for a statutory exclusion.
  8. Submit to the right contractor. Send the claim to the DME MAC for the patient’s state, on a CMS-1500 or an 837P.

Documentation requirements for V2319 claims

An auditor checking V2319 wants to see three things: the segment width, the coverage basis and proof of delivery. Keep each one in the patient record before the claim goes out.

Prescription and lens order

  • A signed prescription showing sphere, cylinder, axis and add for each eye
  • A lens order that names the trifocal style and its segment size, such as 8×35
  • A segment width over 28 mm on every lens billed with V2319

Coverage evidence

  • The operative note for each cataract surgery with IOL insertion, showing the date and the eye
  • Or the documented aphakia or congenital aphakia diagnosis
  • A note that this pair is the first one supplied after that surgery
  • A signed ABN whenever the claim carries the GA modifier

Dispensing record

  • The base lens code and any add-on codes billed for each eye
  • The date the glasses were delivered
  • The patient’s signature or receipt confirming delivery

Pro Tip

Copy the segment size from the lab order onto the dispensing ticket, next to the base lens code. A 7×28 flat-top stays off the claim, while an 8×35 segment gets its own V2319 line.

Common V2319 billing errors and how to fix them

Most V2319 problems come from treating it as a lens rather than a feature of one. The table pairs each error with its fix.

Error What went wrong Fix
V2319 billed alone No base lens line on the claim Add the matching V2300-V2314 or V2399 line and resubmit
Billed for a power range V2319 used to describe sphere and cylinder Code the power with the base lens code and keep V2319 for the segment
Segment of 28 mm or less The descriptor requires a width over 28 mm Remove the V2319 line
Bifocal billed with V2319 The lens has two zones, not three Use V2219 for a wide bifocal segment
Oversize lens coded as V2319 A large lens blank does not widen the segment Use V2780 for an oversize lens, with KX when necessity is documented
Missing laterality No RT or LT, or one RTLT line with two units Bill separate RT and LT lines with one unit each
No covered diagnosis Routine glasses billed to Medicare Part B Add the supported Z96.1, H27.0- or Q12.3 code, or bill the patient or vision plan
Wrong contractor Claim sent to the A/B MAC Resubmit to the DME MAC for the patient’s state

How Pabau keeps V2319 claims tied to the lens order

In many optical practices, the prescription lives in the exam record while the lens order sits with the lab. The claim gets typed up later, and the segment size is often missing by then.

In Pabau’s practice management software, the clinical notes, the prescription and the claim share one patient record. Your team builds the V2319 lines from data already on that record.

Its claims software for billing teams then submits the claim through Claim.MD and tracks its status. Each claim shows as pending, submitted, processing, paid or error on one dashboard. A rejected V2319 line is visible as soon as its status changes, so your biller can correct it while the order is fresh.

Track every V2319 claim from one record

Pabau keeps the prescription, clinical notes and claim on one patient record. Claims are submitted and tracked from the same place, so your team sees each line’s status without switching systems.

Conclusion

V2319 turns on one measurement. It belongs on the claim only when a trifocal segment is wider than 28 mm, and only beside a base lens code. Record the segment size at the dispensing table, where the lab order is already in front of you.

The bigger decision is coverage. For a Medicare patient with no aphakia and no recent IOL surgery, the whole pair falls outside Medicare coverage. Telling the patient before the lenses are ordered protects them from a surprise bill and protects your receivable.

Frequently asked questions

What is HCPCS code V2319?

HCPCS code V2319 is the Level II code for a trifocal lens with a segment width over 28 mm. It reports a wide trifocal segment as an add-on line, next to the base trifocal lens code. It does not describe lens power.

Does Medicare cover V2319?

Only in limited cases. Medicare Part B excludes routine eyeglasses. It covers corrective lenses for aphakia, and one pair of conventional eyeglasses or contact lenses after each cataract surgery with an intraocular lens. V2319 is billed within that benefit when the segment is wider than 28 mm.

Can V2319 be billed without a base lens code?

No. CMS policy treats V2319 as an add-on feature of a trifocal lens. Each lens needs one base code from V2300 to V2314 or V2399, and V2319 goes on a separate line beside it.

Does a 28 mm trifocal segment qualify for V2319?

No. The descriptor requires a segment over 28 mm, so a standard 7×28 flat-top does not qualify. A 35 mm segment or a full-width Executive-style segment does.

What is the difference between V2319 and V2219?

Both report a segment wider than 28 mm. V2319 applies to a trifocal lens and V2219 to a bifocal lens. Each is an add-on billed beside a base lens code from its own block.

What modifiers are used with HCPCS code V2319?

Every V2319 line needs RT or LT. Bill a pair as two lines with one unit each, not one RTLT line with two units. Add GA or GZ when Medicare coverage criteria are not met, depending on whether an ABN is on file.

What ICD-10 codes support a V2319 claim?

For Medicare, the Refractive Lenses policy accepts Z96.1 for presence of an intraocular lens. It also accepts H27.00 to H27.03 for aphakia and Q12.3 for congenital aphakia. Without one of these, routine eyeglasses are statutorily excluded.
