HCPCS code E0272 – Foam rubber mattress billing guide

Code Definition

E0272 is the HCPCS Level II code for a foam rubber mattress used on a hospital bed. Its official descriptor reads "Mattress, foam rubber," and the innerspring version is E0271.

Medicare covers E0272 as a replacement mattress for a hospital bed the beneficiary already owns, under LCD L33820. A mattress delivered with a new bed is bundled into the bed code, such as E0250. Claim lines carry KX when the criteria are met, or GA or GZ when they are not.

Key takeaways

Key takeaways

HCPCS Code E0272 describes a foam rubber mattress for a hospital bed, and its official descriptor is Mattress, foam rubber, with no pad component.

Medicare covers E0272 as a replacement mattress for a hospital bed the beneficiary already owns, under LCD L33820 and policy article A52508.

A mattress delivered with a hospital bed is never billed separately, because with-mattress bed codes such as E0250 and E0260 already include it.

E0272 is an inexpensive or routinely purchased item, so each claim line carries NU, UE or RR plus KX, GA or GZ.

Pressure-reducing support surfaces such as E0185 and E0277 follow their own coverage policies, so a pressure-reduction need is never billed as E0272.

HCPCS Code E0272: official descriptor and code overview

HCPCS Code E0272 is the code for a foam rubber mattress used on a hospital bed. It belongs to the HCPCS Level II code set maintained by the Centers for Medicare and Medicaid Services (CMS). The official descriptor is three words long: “Mattress, foam rubber.”

However, the descriptor names only the material. It also includes no incontinent pad, no overlay, and no pressure-reducing feature. The innerspring version of the same mattress is E0271.

Field Detail
HCPCS code E0272
Long descriptor Mattress, foam rubber
Short descriptor Mattress foam rubber
Code category Hospital beds and accessories
Code type HCPCS Level II, durable medical equipment (DME)
Medicare coverage code D, special coverage instructions apply
Pricing indicator 32, inexpensive and routinely purchased DME
Medicare policy LCD L33820 and policy article A52508
Where claims go The DME Medicare Administrative Contractor (DME MAC) for the beneficiary’s state

First, coverage code D sends you to the hospital bed policy for the rules. Second, pricing indicator 32 sets the payment method, which in turn decides the modifiers on the claim line.

What E0272 covers and how it differs from adjacent codes

E0272 covers a standard foam rubber mattress sized for a hospital bed. In fact, most coding errors on this code come from three neighbors: the innerspring mattress, the beds that already include a mattress, and the pressure-reducing support surfaces.

Code Official descriptor How it relates to E0272
E0271 Mattress, innerspring Same coverage rule and same bundling. Only the mattress construction differs.
E0272 Mattress, foam rubber This code
E0250 Hospital bed, fixed height, with any type side rails, with mattress The bed allowance already includes a mattress, so E0272 is not billed alongside it.
E0251 Hospital bed, fixed height, with any type side rails, without mattress When E0251 and E0272 are delivered together, bill the combined code E0250.
E0185 Gel or gel-like pressure pad for mattress, standard mattress length and width A pressure-reducing support surface with its own coverage policy
E0277 Powered pressure-reducing air mattress A powered support surface with its own coverage policy and nationwide prior authorization

So choose between E0271 and E0272 by the mattress you deliver, not by the diagnosis. If the patient needs pressure reduction, the order describes a support surface, and E0272 is the wrong item category altogether. Next, the decision path below runs those checks in the order a biller should.

Decision path for a hospital bed mattress claim
Bundling and bed ownership rule out more E0272 claims than the mattress material does. Drawn from LCD L33820 and policy article A52508.

Medicare coverage for E0272 under LCD L33820

Medicare covers E0272 as a replacement mattress for a hospital bed the beneficiary owns. Specifically, LCD L33820 (Hospital Beds and Accessories) states the rule in a single sentence. Medicare covers a replacement innerspring or foam rubber mattress for a beneficiary-owned hospital bed when the condition requires it.

As a result, that rule creates three billing situations, and only one of them pays E0272 on its own line.

  • Replacement on an owned bed: the beneficiary owns a covered hospital bed and the mattress needs replacing. Then bill E0272 with KX.
  • Mattress delivered with a new bed: the mattress is part of the bed allowance. Instead, bill the with-mattress bed code, such as E0250, E0255 or E0260, and leave the mattress off the claim.
  • Bed still in its rental period: hospital beds are capped rental items, so the beneficiary does not own the bed yet. Therefore, a separate E0272 claim does not meet the owned-bed rule.

Policy article A52508 lists E0271 and E0272 as items bundled into the bed codes when provided at the same time. It also sets combination rules, such as billing E0250 when the supplier delivers E0251 with a mattress.

The hospital bed itself must still meet coverage criteria

Medicare covers the replacement mattress because it covers the bed. LCD L33820 covers a fixed-height hospital bed when one or more of these applies to the beneficiary:

  • A medical condition requires positioning of the body in ways not feasible with an ordinary bed.
  • The beneficiary needs positioning not feasible in an ordinary bed to alleviate pain.
  • The beneficiary must keep the head of the bed raised more than 30 degrees most of the time, because of congestive heart failure, chronic pulmonary disease, or problems with aspiration.
  • The beneficiary needs traction equipment that attaches only to a hospital bed.

The medical record behind an E0272 claim should show that one of these still applies. However, a mattress for an ordinary household bed falls outside the policy.

Documentation requirements for E0272

Strong medical billing compliance for E0272 rests on four records. Keep each one on file before the claim goes out.

Document What it must show Notes
Standard written order (SWO) Beneficiary name or Medicare number, order date, item description, quantity, and the treating practitioner’s name or NPI and signature Must reach the supplier before the claim is submitted
Medical record The condition that meets a hospital bed criterion, and why the mattress needs replacing Kept by the treating practitioner and made available on request
Bed ownership record The purchase of the hospital bed or the completed capped rental period Shows the replacement is for a beneficiary-owned bed
Proof of delivery Beneficiary name, delivery address, item description, quantity, delivery date, and the signature of the person receiving it Retain for audits and appeals

E0272 needs neither a face-to-face encounter nor a written order prior to delivery. It is absent from the CMS list for both requirements as of the October 28, 2026 update. Some hospital bed codes, such as E0290, E0301 and E0304, are on that list, so check the bed code separately.

Certificates of medical necessity do not apply here either. CMS retired them for DME claims in January 2023, and the standard written order took their place.

ICD-10 codes that support the hospital bed

However, policy article A52508 publishes no list of covered ICD-10-CM codes. Instead, the diagnosis on an E0272 claim is the condition that justifies the hospital bed. The examples below map common diagnoses to the bed criterion they can support, but the medical record has to back each one.

ICD-10-CM code Description Hospital bed criterion it can support
I50.9 Heart failure, unspecified Head of bed raised more than 30 degrees most of the time
J44.9 Chronic obstructive pulmonary disease, unspecified Head of bed raised more than 30 degrees most of the time
R13.10 Dysphagia, unspecified Head elevation for problems with aspiration
G82.20 Paraplegia, unspecified Positioning not feasible in an ordinary bed
G12.21 Amyotrophic lateral sclerosis Positioning not feasible in an ordinary bed
I69.351 Hemiplegia and hemiparesis following cerebral infarction affecting right dominant side Positioning not feasible in an ordinary bed
M62.81 Muscle weakness (generalized) Secondary only. It does not meet a bed criterion on its own

Pressure ulcer codes from the L89 category, for example, do not establish coverage for E0272. They support the separate support surface policies, which cover items such as E0185 and E0277.

Modifiers for E0272

E0272 needs two kinds of modifier on every claim line. The first says how the supplier provided the item, and the second says whether the claim meets the coverage criteria.

Modifier Meaning When to use it on E0272
NU New equipment A new mattress sold to the beneficiary
UE Used durable medical equipment A used mattress sold to the beneficiary
RR Rental The mattress is rented rather than purchased
KX Requirements in the medical policy have been met The beneficiary owns a covered hospital bed and the condition requires a replacement mattress
GA Waiver of liability statement issued as required by payer policy Criteria are not met and a valid Advance Beneficiary Notice (ABN) is on file
GZ Item expected to be denied as not reasonable and necessary Criteria are not met and no ABN was obtained

Policy article A52508 is direct about the second group. Medicare rejects claim lines without KX, GA or GZ as missing information. KX belongs on an accessory only when the accessory meets its own criteria.

In addition, BP, BR and BU do not belong on E0272. Those modifiers report a purchase or rental election for capped rental items, but Medicare pays E0272 as an inexpensive or routinely purchased item.

E0272 payment, the fee schedule and prior authorization

Medicare pays E0272 from the DMEPOS fee schedule, not the physician fee schedule. Because it is an inexpensive or routinely purchased item, you can buy it new, buy it used, or rent it. Still, total rental payments cannot exceed the purchase allowance.

Look up the current allowance in the CMS DMEPOS fee schedule files. CMS publishes the amounts by state and updates them each quarter, so pull the figure for the quarter of the date of service.

Additionally, E0272 is not on the Required Prior Authorization list as of the October 28, 2026 update. However, E0277, the powered air mattress, is on that list nationwide, which is one more reason the two codes are never interchangeable. Medicare Advantage and commercial plans can still require their own authorization.

How to bill E0272: step-by-step

A clean E0272 claim follows seven steps, and you complete most of them before delivering the mattress.

  1. Confirm the bed is beneficiary-owned. First, check the purchase date or the end of the capped rental period. If the bed ships with the mattress, bill the with-mattress bed code instead.
  2. Confirm the material. Foam rubber is E0272 and innerspring is E0271. However, a pressure-reducing surface is a different code under a different policy.
  3. Check the record against the bed criteria. The notes should show the condition that still requires a hospital bed and why the mattress needs replacing.
  4. Get the standard written order. The order must reach the supplier before you submit the claim.
  5. Issue an ABN when the criteria are not met. A signed notice then lets you bill with GA and hold the beneficiary liable.
  6. Build the claim line. Add NU, UE or RR, then KX, GA or GZ. Next, submit to the DME MAC for the beneficiary’s state on the CMS-1500 or 837P.
  7. Keep proof of delivery. Store it with the order and the ownership record, so you can answer audits and appeals.

Common denial reasons for E0272 and how to avoid them

Indeed, most E0272 denials trace back to the bundling rule or the modifier pair. Therefore, effective DME denial management starts with the patterns below.

Denial reason Root cause Prevention or fix
Billed with a new hospital bed E0272 billed on the same date as a with-mattress bed code Bill only the bed code, or the combined code when the bed is a without-mattress version
Rejected for missing information No KX, GA or GZ on the claim line Add the modifier and submit a new claim, since a rejected claim has no appeal rights
Bed not beneficiary-owned The bed is still within its capped rental period Confirm ownership before you deliver a replacement
Medical necessity not supported The record does not show a hospital bed criterion Ask the treating practitioner for notes that document the qualifying condition
Wrong item category A pressure-reducing need billed as E0272 Code the support surface the order describes, under its own policy
No order on file The standard written order arrived after the claim was submitted Hold the claim until the order is in the file

How to appeal a denied E0272 claim

Medicare has five levels of appeal, and a documentation denial usually resolves at the first. However, Medicare does not allow an appeal of a rejected claim, such as one missing KX, GA or GZ, so correct and resubmit it instead. The remittance advice names each reason with CARC and RARC codes, which our guide to medical billing denial codes decodes.

  1. Redetermination: file with the DME MAC within 120 days of receiving the initial determination. Send the order, the medical record and the ownership record.
  2. Reconsideration: a Qualified Independent Contractor reviews the case. Then file within 180 days of receiving the redetermination decision.
  3. Administrative Law Judge hearing: file within 60 days of receiving the reconsideration decision. Also, the claim must meet the amount in controversy that CMS sets each year.
  4. Medicare Appeals Council review: file within 60 days of receiving the hearing decision.
  5. Federal district court review: file within 60 days of the Council’s decision, subject to the court’s amount in controversy.

So weigh the cost of each appeal against the allowance. Understanding the full revenue cycle management impact helps you decide which E0272 denials are worth pursuing past Level 1.

How Pabau keeps E0272 claims documented and tracked

An E0272 claim stands or falls on records collected before delivery. The order, the bed ownership history and the delivery signature often sit in three different places, and the claim waits while someone finds them.

Pabau’s practice management platform keeps those records together on one patient file, so nobody has to hunt for them. For example, digital forms capture the order and the delivery sign-off. Pabau’s claims management software then submits the claim your team has coded and tracks its status through adjudication.

When a denial comes back, the documents an appeal needs are already attached to the patient. Your billers spend their time on the fix rather than on the search.

Keep every replacement mattress claim documented

Pabau holds the standard written order, the bed ownership record and the proof of delivery on one patient record. Your team submits E0272 claims electronically and tracks each one through to payment.

Conclusion

E0272 is a narrow code. It pays for a replacement foam rubber mattress on a hospital bed the beneficiary owns, but for little else. A mattress that ships with a new bed is already paid inside the bed code.

Check two facts before every claim: who owns the bed, and whether the mattress arrived with it. Once you get those right and add the right modifier pair, most E0272 denials never happen. Book a demo to see how Pabau keeps DME orders, delivery records and claim status on one patient record.

Frequently asked questions

What is HCPCS Code E0272?

HCPCS Code E0272 is the Level II code for a foam rubber mattress used on a hospital bed. Its official descriptor reads Mattress, foam rubber, with no pad component. Medicare covers it as a replacement mattress for a hospital bed the beneficiary owns, under LCD L33820.

What is the difference between E0271 and E0272?

E0271 is an innerspring mattress and E0272 is a foam rubber mattress. Both follow the same coverage rule in LCD L33820, and the with-mattress hospital bed codes bundle both. So pick the code that matches the construction of the mattress you deliver.

What modifiers does an E0272 claim need?

First, add NU for a new purchase, UE for used equipment, or RR for a rental. Then add KX when the coverage criteria are met, or GA or GZ when they are not. Medicare rejects claim lines without KX, GA or GZ as missing information.

Can E0272 be billed with a new hospital bed?

No, not when the supplier delivers the bed and mattress together. The allowance for with-mattress bed codes such as E0250 and E0260 already includes the mattress. If the bed is a without-mattress code, bill the single combined code instead, such as E0250 in place of E0251.

Does E0272 require prior authorization or a face-to-face encounter?

No. E0272 is not on the Required Prior Authorization list as of the October 28, 2026 update. It is also absent from the face-to-face and written order prior to delivery list. A standard written order must still reach the supplier before you submit the claim. Medicare Advantage and commercial plans may set their own rules.
