The average cost of Botox per state runs from $12 to $15 a unit, and the national median is $13. Those figures come from the Pabau Botox Price Check, our count of 562 practices’ own published price lists across all 50 states. We collected every price on November 4, 2026.
At that $13 median, the average cost of Botox for a 20-unit frown-line treatment is about $260. Three in four prices sit between $12 and $14, and big-city practices charge the same median as rural ones. Find your state in the table, then work out your total from the units you need.
Key takeaways
The national median list price for Botox Cosmetic is $13 a unit, across 562 practices’ own published price lists.
Three in four prices (74%) fall between $12 and $14 a unit, and only 15% are $15 or more.
Connecticut, Hawaii and Oregon have the highest state medians at $15 a unit, while 13 states share the lowest at $12.
The coastal premium cost guides describe doesn’t show up, because metro and non-metro practices both have a $13 median.
Practices that publish a membership or new-patient price charge about $2 a unit less than their own list price.
Average cost of Botox by state
State medians for Botox run from $12 to $15 a unit. Of the 47 states with enough published prices to rank, 43 sit between $12 and $14, and 19 sit at exactly $13.
The average cost of Botox by state is flatter than most cost guides suggest. The average cost of Botox in California, for example, is $13 a unit, the same as Texas, Michigan and Ohio. The chart groups every ranked state by its median, so you can see how tightly they bunch.

The table lists every state. The median is weighted by where people live in the state, and the typical range shows the middle half of the prices we found there.
| Rank | State | Median price per unit (population-weighted) | Typical range (middle half of prices) | Practices sampled |

States where Botox costs the most
Connecticut, Hawaii and Oregon have the highest Botox prices, each with a median of $15 a unit. Pennsylvania follows at $14.50, then seven states at $14.
Those seven are Iowa, Kansas, Maine, Maryland, New Hampshire, New York and Virginia. In Connecticut, the middle half of all prices sits at exactly $15. Even in the priciest states, the highest list price we found anywhere was $18 a unit.
New York shows why the weighting matters. Its raw median is $15, but weighting each region by its population brings the state to $14.
States where Botox costs the least
Thirteen states share the lowest median, $12 a unit. The 13 are Colorado, Florida, Idaho, Illinois, Indiana, Montana, Nebraska, New Mexico, South Dakota, Tennessee, Utah, West Virginia and Wyoming.
Arizona and South Carolina sit just above them at $12.50. The cheapest group includes two of the most populous states, Florida and Illinois, so low prices are not a small-town effect. Across all 562 practices, 9% charge under $12, and the lowest list price was $8.99.
The coastal premium that doesn’t show up
Cost guides commonly say Botox costs $15 to $25 a unit on the coasts and in big cities, and $10 to $15 in rural areas. None of the guides we read names a sample for those ranges, and the Pabau Botox Price Check does not reproduce them.
- Only 15% of the 562 published prices are $15 or more, and none is above $18.
- Practices inside a metro area have a median of $13 (491 practices). Practices outside every metro area also have a median of $13 (71 practices), with the same $12 to $14 middle range.
- California, Massachusetts, New Jersey and Washington all have a $13 median, the same as Alabama and Oklahoma.
- Connecticut and Hawaii do sit at $15, and Maryland and New York at $14, so a few coastal states run higher. Florida, a coastal state, is at $12.
So a quote of $18 a unit in a big city is above what most practices in any state publish. Location explains far less of the price than the practice you choose.
States without enough published prices
Vermont, Delaware, Nevada and Washington, DC are not ranked, because fewer than eight practices there publish a per-unit Botox price that met our rules.
Vermont and Delaware each have six usable prices, Nevada has seven and Washington, DC has three. Their medians appear at the foot of the table, labeled as not ranked, but treat them as rough indications.
Nevada works differently. Several Nevada practices say they can only sell Botox by the 50-unit vial, so most publish a vial price instead. The six vial prices we found run from $500 to $800 for 50 units, with a median of $600, or about $12 a unit.
How much is Botox per unit?
Botox costs $13 per unit at the median US practice, and about three in four practices charge between $12 and $14. The published list prices we found ran from $8.99 to $18 a unit.
So how much is Botox per unit near you? Usually a round number. The average price per unit of Botox lands on whole dollars at most practices, and six prices cover most of the market:
- $13 a unit: 137 practices
- $12 a unit: 132 practices
- $14 a unit: 102 practices
- $15 a unit: 62 practices
- $11 a unit: 22 practices
- $10 a unit: 21 practices
Prices from $12 to $14 account for 74% of the sample. This Botox cost per unit is the regular list price for Botox Cosmetic. Membership, new-patient and sale prices were recorded separately and kept out of the median.
A per-unit price only answers half of how much Botox costs, because the units decide the total. A practice charging $11 a unit can still cost more than one at $13 if it uses more units for the same area.
How much does Botox cost per treatment?
A Botox treatment costs about $260 for frown lines (20 units) and about $832 for a full upper face (64 units) at the $13 median.
The doses below are the ones on the FDA-approved Botox Cosmetic label. The prices are simple arithmetic at $12, $13 and $14 a unit, the range most practices charge. They show average Botox costs rather than any single price list.
| Units | What that dose treats (FDA label) | At $12 a unit | At $13 a unit | At $14 a unit |
|---|---|---|---|---|
| 20 | Frown lines between the brows (glabellar lines) | $240 | $260 | $280 |
| 24 | Crow’s feet, 12 units per side (lateral canthal lines) | $288 | $312 | $336 |
| 40 | Forehead lines plus frown lines, dosed together | $480 | $520 | $560 |
| 44 | Crow’s feet plus frown lines | $528 | $572 | $616 |
| 50 | One 50-unit vial, often sold whole in Nevada | $600 | $650 | $700 |
| 64 | Full upper face: forehead, frown lines and crow’s feet | $768 | $832 | $896 |
How much is 40 units of Botox? About $520 at the median, which is the labeled dose for forehead and frown lines together. How much does full face Botox cost? The label covers the upper face at 64 units, or about $832. Lower-face areas such as the jaw are off-label, so their doses vary by injector.
The label also covers neck bands (platysma bands) at 26, 31 or 36 units, depending on how many bands are treated. Those doses cost about $338 to $468 at $13.
How much does Botox cost for forehead lines?
Botox for forehead lines costs about $520 at the $13 median, because the FDA label doses 20 forehead units together with 20 frown-line units.
So how much does Botox cost for forehead lines alone? Some practices price the forehead as a flat area instead. Only three practices in the Pabau Botox Price Check publish a flat forehead price, with a median of $200. At $13 a unit, $200 buys about 15 units, which is below the labeled 20.
Per-area prices are rare on price lists, so treat them as examples. The few we found had a median of $240 for frown lines and $220 for crow’s feet, from three practices each. Seven practices list a lip flip, at a median of $150.
Masseter Botox cost
Masseter Botox cost depends on the dose. At the $13 median, every 10 units adds $130, so 25 units per side costs $650 and 50 units per side costs $1,300.
Jaw slimming and teeth-grinding treatment in the masseter muscle are off-label, because neither Botox label includes them. A 2025 review in the journal Toxins found no settled consensus on the average masseter dose. Cost guides quote doses from 15 to 80 units per side, so ask your injector for the unit count before you compare quotes.
Very few practices in the Pabau Botox Price Check publish a flat masseter price, too few to report a median.
Membership and new-patient prices: What they save
Practices that publish a membership price charge about $2 a unit less than their own list price, a median of $11.50 against $13.50. New-patient and promotional prices save the same $2.
| Discounted price type | Practices publishing both prices | Median list price | Median discounted price | Saving per unit |
|---|---|---|---|---|
| Membership | 115 | $13.50 | $11.50 | $2.00 |
| New-patient, promo or special | 59 | $13.00 | $10.99 | $2.00 |
These comparisons come from the same practice, so the saving is not a side effect of comparing a cheap state with an expensive one. The promo median of $10.99 is a good example of charm pricing, and it lands just under a round number.
A $2 saving adds up across a year. The Botox Cosmetic label puts the effect on frown lines at about three to four months. Its safety data do not cover retreatment more often than every three months. So a year of upkeep means three or four sessions.
- Frown lines, 20 units: $780 to $1,040 a year at $13 list price. A $2 member discount saves $120 to $160 of that.
- Full upper face, 64 units: $2,496 to $3,328 a year at $13. The same discount saves $384 to $512.
Manufacturer loyalty programs can lower the cost further, such as Allē for Botox and ASPIRE for Dysport. For injectors weighing a plan of their own, our guide to a med spa membership program covers pricing the tiers.
Botox vs Dysport, Xeomin, Jeuveau and Daxxify prices
Xeomin, Jeuveau and Daxxify each have a $12 median per unit on practices’ own price lists, a dollar below Botox. At practices that list both, the other toxin is usually the same price or cheaper.
| Brand | Median list price per unit | Practices listing it | Compared with Botox at the same practice |
|---|---|---|---|
| Botox Cosmetic | $13.00 | 562 | Baseline |
| Xeomin | $12.00 | 182 | Same price at 70 of 148 practices, cheaper at 78, pricier at none |
| Jeuveau | $12.00 | 133 | Same price at 36 of 99 practices, cheaper at 63, pricier at none |
| Daxxify | $12.00 (middle half $8 to $14) | 116 | Same price at 33 of 98 practices, cheaper at 51, pricier at 14 |
| Dysport | $4.66 | 145 | Cheaper per unit at all 130 practices, but its units are not Botox units |
The Xeomin price per unit is a useful benchmark if you want a cheaper toxin. Xeomin was never pricier than Botox at the 148 practices that list both. Daxxify is the only brand that sometimes costs more, at 14 of 98 practices.
Dysport looks far cheaper at $4.66 a unit, but its units are measured differently. The Botox Cosmetic label states that its units cannot be compared to or converted into the units of any other botulinum toxin. Practices dose Dysport in more units for the same area, so compare the total quote, not the unit price.
What changes the price more than your state
The number of units you need, the type of practice, the injector and any membership price each move your bill more than your state does.
- Units: 20 units for frown lines costs $260 at the median, while 64 units for the full upper face costs $832. The dose matters far more than the state.
- Practice type: med spas have a $13 median (503 practices), and plastic surgery practices also sit at $13 (26). Dermatology practices sit at $15, but only 13 published a price, so treat that as a hint.
- Who injects: some practices publish a different per-unit price depending on who injects. We recorded the lowest tier, so a physician-only appointment may cost more.
- Membership or promo: about $2 a unit off list, as shown above.
Prices also vary more inside some states than between them. In New York the middle half of prices runs from $13 to $16, while North Carolina’s middle half sits at exactly $13. Rules on who can inject Botox differ by state, which shapes who you will meet at the appointment.
How we collected the prices
We checked 2,986 practice websites across all 50 states, and 609 of them showed a Botox per-unit list price at all. Of those, 562 met every rule and make up the Pabau Botox Price Check. All pages were fetched and archived on November 4, 2026.
- Own price lists only: each figure comes from the practice’s own website. Review sites, marketplaces, news stories and modeled figures were never used.
- One row per location: each practice location counts once, and a chain with one price list counts once per state.
- List price only: membership, new-patient and sale prices were recorded separately. Volume tiers were read at the tier covering 20 units, and injector tiers at the lowest price.
- 47 prices excluded: 16 were worked out from a package, and 14 were a shared “starting at” price for several brands. Another 7 were dated before 2025, 4 sat only in page code and 6 failed other checks.
- Weighting: each state median is weighted by population across its largest metro, its other metros and its non-metro areas, using the OMB 2023 metro delineations.
- Ranking: a state needs at least eight usable prices to be ranked. The target was 10 practices per state, and 20 in the 10 most populous.
- Checks: 161 rows were re-checked from scratch, including random samples and every shared-price row.
Weighting moved 17 states by 50 cents or more. Pennsylvania rose $1.12 to $14.50, while New York, Massachusetts, Colorado, Montana and Arkansas each fell $1.
The sample has limits. About 10 prices per state is enough to place a state, not to rank two neighbors precisely. It also covers published prices only, and many practices quote Botox only at a consultation.
Publishing a Botox price list patients can trust
Patients shopping for Botox want a per-unit number, and most practices we checked never publish one. If you run a practice, a clear list price, a member price and an honest unit estimate answer the first question every patient asks.
Pabau, the medical spa software we build, keeps those prices consistent from booking to invoice. Patients book online against the treatments you set up, and the same treatment drives the invoice. Pabau’s memberships feature bills members weekly, monthly or yearly and applies their discount automatically, so the $2 member saving happens without a manual adjustment.
Pabau also logs the units and batch number of each injection against the treatment note and deducts the product from stock. Your unit counts and stock stay in step with what each patient paid. Test any price change against our guide to choosing a med spa pricing strategy.
Keep Botox pricing consistent at every visit
Pabau ties online booking, membership billing and unit-level stock tracking to the same treatment record. Patients pay the price they saw, and every unit is accounted for.

Conclusion
Your state barely moves the price of Botox. Most practices charge $12 to $14 a unit, whether they sit on the coast or in a small town.
The bigger levers are the units you need and the practice you pick. Ask for the unit count, multiply it by the per-unit price, and check whether a membership takes $2 off each unit. If a quote runs above $15 a unit, it is above what 85% of practices publish.
For practice owners, the same data is a benchmark. A published price close to $13 with a clear member rate is easy for patients to trust. Book a demo to see how Pabau keeps your Botox pricing, memberships and unit tracking in one place.

Frequently asked questions
How much is 50 units of Botox?
A 50-unit Botox treatment costs about $650 at the $13 national median, and $600 to $700 across the middle half of practices. In Nevada, where several practices sell only by the 50-unit vial, published vial prices run from $500 to $800, with a $600 median.
How much does Botox cost each visit?
A typical visit costs $260 to $832 at the $13 median. Average Botox costs sit at the low end for 20 units on frown lines. They reach the top for 64 units across the full upper face.
How much does Botox cost with insurance?
Cosmetic Botox costs the same with insurance as without, because health plans generally exclude cosmetic use. Aetna’s botulinum toxin policy, for example, calls cosmetic use ineligible. Plans may pay for medical uses on the Botox label, usually after prior authorization. Those include chronic migraine, severe underarm sweating, cervical dystonia and overactive bladder. The label defines chronic migraine as 15 or more headache days a month, per the Botox prescribing information.
How much does armpit Botox cost?
Underarm Botox costs about $1,300 at the $13 median, because the Botox label doses 50 units per armpit, or 100 units in total. Severe underarm sweating is a labeled medical use, so insurance may cover part of the cost after prior authorization.
How long does Botox last?
Results on frown lines wear off after about three to four months, according to the Botox Cosmetic label. A year of upkeep therefore takes three or four treatments, or $780 to $1,040 for frown lines at the $13 median.
