HCPCS code E0315 – Bed accessory billing guide

Code Definition

HCPCS code E0315 is the Level II code for a bed accessory: board, table, or support device, any type. Medicare lists it as noncovered, so the patient pays unless another payer covers the item.

Policy article A52508 calls bed boards and over-bed tables billed as E0315 not primarily medical in nature. Bed side rails take their own codes, E0305 and E0310. Bill Medicare with the GY modifier only when a secondary payer needs a denial.

Key takeaways

Key takeaways

HCPCS code E0315 is a bed accessory: board, table, or support device, any type. It is not a bed rail code.

Medicare lists bed boards and over-bed tables billed as E0315 as noncovered, because they are not primarily medical in nature.

Half-length side rails bill as E0305 and full-length side rails as E0310, under their own coverage rules.

E0315 has no Medicare fee, so quote the patient your own price and get their written agreement before delivery.

Append the GY modifier only when a secondary payer needs a Medicare denial before it pays.

HCPCS code E0315 covers bed boards, tables and support devices

HCPCS code E0315 describes a bed accessory that is a board, a table or a support device of any type. Medicare treats it as noncovered. It sits in the E0250-E0373 range for hospital beds and accessories, one section of the full HCPCS Level II list.

Attribute Detail
HCPCS code E0315
Official descriptor Bed accessory: board, table, or support device, any type
Short descriptor Bed accessory brd/tbl/supprt
Code type HCPCS Level II, durable medical equipment (E codes)
Code group E0250-E0373, hospital beds and accessories
Medicare coverage code M, non-covered by Medicare
Pricing indicator 00, not separately priced by Part B
Coverage policy LCD L33820 and policy article A52508, Hospital Beds and Accessories

The “any type” wording makes E0315 a catch-all for accessories used with a bed. It names three kinds of item: a board, a table and a support device. It says nothing about rails, mattresses or the bed itself.

The coverage and pricing values above come from the October 2026 alpha-numeric file on the CMS HCPCS quarterly update page. Check the newest quarterly file before you bill, since CMS can change either value.

E0315 vs E0273, E0274, E0305 and E0310: Pick the right bed code

E0315 is often confused with the bed rail codes, because the codes sit next to each other. Rails have their own codes and a Medicare coverage route. E0315 describes no rail and has no coverage route.

Code Official descriptor Medicare status under the hospital beds policy
E0273 Bed board Noncovered, not primarily medical in nature
E0274 Over-bed table Noncovered, not primarily medical in nature
E0305 Bed side rails, half length Covered when the patient’s condition requires them and they are an integral part of, or an accessory to, a covered hospital bed
E0310 Bed side rails, full length Same rule as E0305
E0315 Bed accessory: board, table, or support device, any type Noncovered when the item is a bed board or an over-bed table
E0316 Safety enclosure frame/canopy for use with hospital bed, any type Covered only when the policy’s criteria are met

Pick the code from the product, not from the diagnosis. A rail of either length is never E0315. When E0273 or E0274 names the item exactly, the more specific code is usually the better choice. Where a product has a PDAC coding decision, follow it.

Sorted by item, the five codes split into two groups, and only the rail group can be paid by Medicare.

Table of bed item codes and Medicare status
Rails are the only bed items here that Medicare can pay for, so a board or table never needs a coverage argument. Statuses from CMS policy article A52508 and the October 2026 HCPCS file.

For a rail, our guides to E0305 and E0310 cover when Medicare pays for one on a covered hospital bed.

Pro Tip

If an order or claim template describes E0315 as a bed rail, fix the template first. Bill half rails as E0305 and full rails as E0310, then check whether the bed code already includes rails.

Why Medicare won’t pay for E0315

The Hospital Beds and Accessories policy article (A52508) says it plainly. A bed board billed as E0273 or E0315 “is noncovered since it is not primarily medical in nature.” An over-bed table billed as E0274 or E0315 is noncovered for the same reason.

That article supports LCD L33820, which the DME MACs publish for all four jurisdictions. The reasoning follows the Medicare definition of DME. Equipment must primarily serve a medical purpose, and a board or table is useful to people who are not ill.

Covered under the hospital beds policy Not covered
Side rails (E0305, E0310) when the patient’s condition requires them A bed board or over-bed table billed as E0315
Safety enclosure (E0316) when the policy’s criteria are met Bed board (E0273)
Replacement innerspring or foam rubber mattress (E0271, E0272) for a beneficiary-owned hospital bed Over-bed table (E0274)
Bed cradle (E0280) when needed to keep bedding off the patient Trapeze bars (E0910, E0911) used on an ordinary bed

The HCPCS file agrees. It gives E0315 coverage code M, non-covered by Medicare, rather than a code that points to special coverage instructions. So no diagnosis, order or modifier changes the Medicare outcome.

Modifiers that apply to E0315

On a noncovered item, modifiers tell Medicare you expect the denial. Two do the work on E0315, and three others don’t belong on the line.

Modifier Description When to use it on E0315
GY Item or service statutorily excluded, or does not meet the definition of any Medicare benefit The standard choice. It fits an item that fails the DME definition.
GX Notice of liability issued, voluntary under payer policy Add it with GY when you gave the patient a voluntary notice.
KX Requirements in the medical policy have been met Never. A noncovered item has no coverage criteria to meet.
GA Waiver of liability statement issued as required by payer policy Not used. GA applies when an item may be denied as not reasonable and necessary.
GZ Item expected to be denied as not reasonable and necessary, no ABN on file Not used, for the same reason as GA.

GY is the one that matters. It produces a clean benefit denial, which is what a secondary payer usually wants to see. An ABN isn’t mandatory for an item Medicare never covers, although many suppliers issue a notice voluntarily.

What the fee schedule says about E0315

Medicare sets no payment for E0315. Its pricing indicator is 00, which means Part B doesn’t price the code separately. There is no allowed amount, so there is no 80% Medicare share and no 20% coinsurance.

The patient owes your full charge unless another payer covers the item. Set a cash price from your cost, and quote it before delivery.

Which payers may cover a bed accessory

Noncovered by Medicare isn’t the same as never paid. Check the patient’s other coverage before you call the item self-pay.

  • Medicaid: state programs write their own DME lists. Check the state’s DME manual or fee schedule, and any prior authorization rule.
  • Medicare Advantage: plans must cover what Original Medicare covers, and may add extra benefits. Check the member’s Evidence of Coverage.
  • Commercial plans: coverage follows each plan’s medical policy. Many treat boards and tables as comfort items.
  • Self-pay: most patients pay for the item themselves. Quote the price in writing first.

A quick insurance eligibility verification step saves a delivery against a benefit that doesn’t exist.

Documentation to keep on file for E0315

Medicare offers no documentation route to payment for E0315. The paperwork still matters, for other payers and for the patient’s own bill.

  • Written order: signed and dated by the treating practitioner, naming the item. Other payers often ask for it.
  • Reason for the item: the chart note that explains why the prescriber ordered the board, table or support device.
  • Signed patient notice: a voluntary ABN or a written cost agreement, signed before delivery.
  • Proof of delivery: a signed receipt listing the item and the HCPCS code.
  • Payer checks: the secondary payer’s policy and any authorization number, saved with the claim.

Which ICD-10 codes belong on an E0315 claim?

Use the diagnosis that explains why the prescriber ordered the item, coded to the highest specificity the chart supports. No ICD-10-CM code makes E0315 payable by Medicare. The diagnosis only documents the order for a payer that reviews it.

How an E0315 claim moves from order to denial

A claim for a bed accessory follows a short, predictable path.

  1. The prescriber orders the item and records the reason in the chart.
  2. Your team confirms the item is a board, table or support device, not a rail.
  3. You check whether E0273 or E0274 describes the item more exactly.
  4. You tell the patient in writing that Medicare won’t pay, and you quote your price.
  5. If a secondary payer needs a denial, you submit the line with GY, plus GX for a voluntary notice.
  6. The DME MAC for the patient’s state denies the line, and the patient gets a Medicare Summary Notice.
  7. You send the denial to the secondary payer, or collect payment from the patient.

Common E0315 mistakes that cost you time

Most E0315 problems come from treating it as a covered rail code.

  • Billing a bed rail as E0315. Half rails are E0305 and full rails are E0310.
  • Expecting payment. Medicare lists boards and over-bed tables under E0315 as noncovered.
  • Appending KX. KX attests that coverage criteria are met, and none exist for this item.
  • Using GA or GZ. Those flag a medical necessity denial. A noncovered item takes GY.
  • Hunting for a qualifying diagnosis. No ICD-10-CM code changes the Medicare outcome.
  • Quoting a Medicare rate. E0315 has no fee, so quote your own price.

For wider patterns across DME lines, a structured denial management process helps.

Keep bed accessory notices, orders and payments on one record

A noncovered item creates front-desk work rather than claim work. Someone has to quote a price, collect a signature and take the payment. In Pabau’s practice management software, the quote, the signature and the payment sit on one patient record.

Pabau’s digital forms store the signed cost notice and the order before the item ships. When a secondary payer needs a denial, Pabau’s claims management lets your team submit the claim and track its status.

Checkout closes the loop. The item gets invoiced and paid at the counter, so the balance doesn’t sit in accounts receivable.

Pabau’s checkout invoices a self-pay item and records the payment, so a noncovered E0315 charge is settled before the patient leaves.

Keep patient notices and self-pay charges on one record

Pabau stores signed cost notices on the patient record and takes payment at checkout. When a secondary payer needs a denial, your team can submit and track the claim.

Conclusion

Treat E0315 as a self-pay item from the first conversation. It isn’t a rail code, and Medicare won’t pay for the boards and tables it describes.

Your effort belongs in two decisions. First, confirm the item really is a board, table or support device, and not a rail billed under E0305 or E0310. Second, decide whether a secondary payer needs a GY denial before you bill Medicare at all.

Get those right, and the claim stops creating rework. Book a demo to see how Pabau keeps signed patient notices and payments ready for every payer request.

Frequently asked questions

What is HCPCS code E0315?

HCPCS code E0315 is the Level II code for a bed accessory: board, table, or support device, any type. It sits in the E0250-E0373 hospital beds and accessories range. Medicare lists it as noncovered.

Is E0315 a bed rail code?

No. Half-length bed side rails bill as E0305 and full-length side rails as E0310. E0315 covers boards, tables and support devices used with a bed, which Medicare treats differently from rails.

Does Medicare pay for E0315?

No. Policy article A52508 lists bed boards and over-bed tables billed as E0315 as noncovered, because they are not primarily medical in nature. The code has no Medicare fee, so the patient pays unless another payer covers it.

What modifier do I use with E0315?

Use GY, which reports an item that meets no Medicare benefit definition. Add GX when you gave the patient a voluntary notice. Don’t use KX, GA or GZ, since those relate to coverage criteria or medical necessity.

Should an over-bed table be billed as E0315 or E0274?

E0274 names an over-bed table exactly, so it is usually the better choice. Policy article A52508 lists the table under both codes, and both lead to the same Medicare denial. Follow any PDAC coding decision for the product.

Does Medicaid or a Medicare Advantage plan cover E0315?

It depends on the plan. Medicaid programs write their own DME lists, and Medicare Advantage plans can add extra benefits. Check the state DME manual or the member’s Evidence of Coverage before delivery.
