Key Takeaways

Software covered: 1. Klarna, 2. Afterpay, 3. PayPal Pay Later, 4. Sezzle, 5. Zip, 6. Splitit, 7. Pabau (for healthcare and aesthetic clinics)

Affirm alternatives range from zero-interest 4-payment apps (Afterpay, Sezzle) to flexible 36-month plans (Klarna) and credit-card-based installments (Splitit)

Aesthetic and medical clinics need BNPL tools that work inside their practice management workflow, not bolted-on retail checkout apps

Pabau’s native Klarna integration keeps patient financing inside the clinical booking and payment workflow, avoiding a separate checkout layer

Affirm is well-known, but it is not the right fit for every merchant. Merchant fees can reach 30% on certain financing products, repayment terms are rigid depending on the product chosen, and healthcare providers often find that retail-first BNPL platforms do not map cleanly onto clinical billing workflows.

These affirm alternatives cover the full range: zero-interest short-cycle apps, flexible long-term financing, and clinic-specific payment tools.

This guide is written for clinic owners, practice managers, and merchants evaluating buy now, pay later options. Each product is assessed on merchant fees, credit check policy, repayment terms, and suitability for healthcare and aesthetic clinic contexts.

Name Best For Standout Feature Starting Price Rating
Klarna Merchants wanting flexible repayment options Pay in 30 days, Pay in 3, or 6-36 month financing ~3-5% + fixed fee per transaction 4.1/5 (210)
Afterpay Zero-interest installment purchases 4 payments over 6 weeks, no consumer interest ~4-6% + fixed fee per transaction 4.3/5 (180)
PayPal Pay Later Merchants already using PayPal Pay in 4 or Pay Monthly up to 24 months Standard PayPal transaction rates apply N/A
Sezzle Small and mid-size merchants 4 payments over 6 weeks, credit-building option ~5-6% + fixed fee per transaction 4.2/5 (65)
Zip No hard credit check shoppers Soft or no hard inquiry policy Varies by product tier 3.8/5 (45)
Splitit High-ticket purchases on existing credit cards No new credit issued; uses existing card limit Negotiated merchant rates 4.4/5 (35)
Pabau Aesthetic and medical clinics Native Klarna integration inside clinic PMS From $65/month 4.7/5 (600+)

1. Klarna: Best affirm alternative for flexible repayment

Klarna is one of the most widely accepted buy now, pay later platforms globally, with a merchant network spanning 500,000+ retailers. Unlike Affirm’s single-lender model, Klarna offers four distinct products under one integration: Pay in 30 days, Pay in 3 installments, longer-term 6-36 month financing with APR, and a one-time card. That product range is its biggest advantage over simpler affirm alternatives.

For merchants, Klarna charges a merchant discount rate typically in the range of 3-5% plus a fixed per-transaction fee, according to Klarna’s merchant documentation. Exact rates are negotiated based on volume and product mix. Klarna performs a soft credit check on consumers, so it does not affect buyers’ credit scores at application.

Key features

  • Pay in 30 days: Interest-free deferred payment for consumers
  • Pay in 3: Three equal installments over 60 days, zero interest
  • Financing (6-36 months): Longer-term plans with APR, ideal for higher-ticket purchases
  • One-time card: Virtual card for one-time use at any merchant
  • Merchant portal: Analytics, order management, and dispute handling
  • Klarna Docs API: Developer documentation for custom checkout integration

Pricing

Klarna does not publish a universal rate card. Merchant fees are quoted individually and depend on transaction volume, average order value, and the specific Klarna products enabled. Expect approximately 3-5% plus a fixed fee as a starting reference, with lower rates possible at higher volumes.

Where Klarna shines

  • Broadest product range among affirm alternatives: short-cycle, medium, and long-term financing in one integration
  • Soft credit check only at application, no hard inquiry on consumers
  • Strong merchant portal with order and dispute management tools
  • Native integration available in Pabau for aesthetic and medical clinics (see Klarna integration below)

Where Klarna falls short

  • Consumer-facing Trustpilot rating of 2.6/5 from 95,000+ reviews suggests significant customer experience issues
  • Merchant support quality flagged repeatedly in Capterra reviews: “Poor merchant support response times”
  • Not designed for clinical or healthcare billing workflows out of the box

Customer reviews

Klarna scores 4.1/5 on Capterra from 210 reviews. Positive themes: wide acceptance, flexible payment options. Negative: high merchant fees and consumer debt concerns cited frequently. According to G2 reviewers (3.9/5, 140 reviews), “easy consumer checkout” is the most praised attribute, while merchant support is the most common criticism.

Who Klarna is best for

  • E-commerce merchants wanting all repayment term lengths in one BNPL provider
  • Aesthetic clinics using Pabau that want a native patient financing option
  • Higher average-order-value retailers where 30-day or financing products suit purchase size

2. Afterpay: Best affirm alternative for interest-free installments

Afterpay built its entire model around one thing: zero interest for consumers, four equal payments over six weeks. That simplicity is both its strength and its limitation. There is no credit check for most purchases, no long-term financing, and no interest charged to the buyer. Merchant fees cover the model, typically around 4-6% plus a fixed per-transaction fee, per Afterpay’s merchant site.

Square acquired Afterpay in 2022, so merchants already using Square’s point-of-sale infrastructure get a relatively seamless integration path. For clinics, though, the 6-week repayment cycle is short relative to the cost of many aesthetic treatments.

Key features

  • 4 installments over 6 weeks: Fixed, interest-free structure
  • No consumer interest: Ever, for standard purchases
  • No credit check: Does not perform a hard inquiry on buyers
  • Square integration: Native POS support for Square merchants
  • In-store mode: QR code-based checkout for physical retail

Pricing

Afterpay charges merchants approximately 4-6% plus a fixed fee per transaction, making it slightly more expensive than some general-retail BNPL alternatives. Merchants recover this through higher conversion rates and larger average order values, which Afterpay reports in its merchant documentation.

Where Afterpay shines

  • Zero consumer interest, simple 4-payment structure that patients and customers find easy to understand
  • No credit check means fewer declined applications at checkout
  • Strong brand recognition in fashion and beauty retail

Where Afterpay falls short

  • 6-week repayment cycle is too short for many higher-cost medical and aesthetic treatments
  • No long-term financing product (12+ months) available
  • Late fees apply to missed payments, creating risk for patients managing healthcare budgets

Customer reviews

Afterpay scores 4.3/5 on Capterra from 180 reviews. The most praised attribute is the straightforward 4-payment model. The most common criticism across Capterra and G2 (4.0/5, 95 reviews): “Not suitable for large purchases” and limited repayment flexibility.

Who Afterpay is best for

  • Retail and beauty merchants with lower average order values (under $500)
  • Square merchants wanting a native BNPL option
  • Businesses where short-cycle repayment aligns with purchase timelines

3. PayPal Pay Later: Best affirm alternative for merchants already using PayPal

PayPal Pay Later is not a standalone BNPL app. It is an extension of the existing PayPal checkout, available as Pay in 4 (four payments over 6 weeks, zero interest) and Pay Monthly (longer-term financing up to 24 months with APR). The appeal is straightforward: if your business already processes through PayPal, there is no separate BNPL integration to manage.

Merchant fees follow PayPal’s standard transaction rates, which means Pay in 4 has no extra cost beyond what merchants already pay PayPal. Pay Monthly carries separate financing fees. PayPal does not perform a hard credit check for Pay in 4, but Pay Monthly involves a more complete credit review.

Key features

  • Pay in 4: four equal payments over 6 weeks, no interest, no extra merchant fee
  • Pay Monthly: 6-24 month financing with APR, separate financing fees apply
  • No separate integration: available inside existing PayPal checkout
  • Consumer familiarity: PayPal’s 400M+ active accounts reduce checkout friction

Pricing

Pay in 4 uses standard PayPal transaction rates (typically 2.99% + $0.49 for card transactions). Pay Monthly involves separate financing fees charged to merchants. Check PayPal’s current merchant pricing page for up-to-date rates by market.

Where PayPal Pay Later shines

  • Zero additional integration for PayPal merchants: activates inside existing checkout
  • Consumer trust built on PayPal’s existing brand and account base
  • Pay Monthly offers longer repayment terms (up to 24 months) that Afterpay and Sezzle cannot match

Where PayPal Pay Later falls short

  • Only practical for merchants already committed to PayPal; switching costs are significant if not
  • Pay Monthly requires a more complete credit check, which can deter healthcare patients
  • No clinic-native integration; still requires a separate checkout flow from practice management systems

Who PayPal Pay Later is best for

  • Merchants deeply embedded in the PayPal ecosystem
  • Businesses that want BNPL without adding a new vendor relationship
  • Retailers where 6-week or up-to-24-month terms cover the purchase range

4. Sezzle: best affirm alternative for small merchants

Sezzle operates a four-payment, six-week interest-free model that mirrors Afterpay’s structure. Its differentiator is a credit-building option called Sezzle Up, which lets consumers opt in to have their payment history reported to credit bureaus. For clinics that serve patients who may be actively building credit, this is a meaningful feature that Afterpay does not offer.

Sezzle targets smaller merchants specifically, with a straightforward onboarding process. Merchant fees run slightly higher than Klarna or Afterpay on average, around 5-6% plus a fixed fee, per Sezzle’s published documentation.

Key features

  • 4 payments over 6 weeks: interest-free, no hard credit check
  • Sezzle Up: optional credit-building reporting to bureaus
  • Simple merchant setup: designed for smaller merchant volumes
  • Sezzle Virtual Card: allows use at merchants not directly integrated

Pricing

Sezzle charges merchants approximately 5-6% plus a fixed fee per transaction. This is at the higher end among the affirm alternatives covered here. There is no monthly platform fee for standard merchant accounts.

Where Sezzle shines

  • Credit-building option distinguishes it from every other BNPL platform in this comparison
  • Low barrier to merchant onboarding, suitable for small business and clinic environments
  • No hard credit check at application

Where Sezzle falls short

  • Smaller merchant acceptance network than Klarna or Afterpay
  • Higher merchant fees than most affirm alternatives
  • No long-term financing product for higher-cost treatments

Customer reviews

Sezzle scores 4.2/5 on Capterra from 65 reviews and 4.1/5 on G2 from 55 reviews. Positive themes: good for smaller merchants and the credit-building option. Negative: limited acceptance network and higher fees are the most common complaints.

Who Sezzle is best for

  • Small and mid-size merchants wanting a straightforward BNPL setup
  • Clinics serving patients who want credit-building benefits from their payment plan
  • Businesses with lower transaction volumes where Klarna’s volume-based pricing advantages do not apply

5. Zip: Best affirm alternative for buy now pay later no credit check

Zip (formerly Quadpay in the US) positions itself around accessibility. Its soft-check or no-hard-inquiry policy, depending on product tier, makes it an option for consumers who are cautious about their credit score or have been declined by lenders that perform hard inquiries. For healthcare providers, that accessibility can matter: patients avoiding a hard credit pull are more likely to complete checkout.

Zip’s merchant fees and product structures vary by market and product tier, and the platform is less well established in the US than Klarna or Afterpay. Its merchant acceptance network is growing but smaller than the category leaders.

Key features

  • Soft or no hard credit inquiry: varies by product tier, per Zip’s help documentation
  • Flexible product tiers: Zip Pay and Zip Money with different credit limits and terms
  • Growing merchant network: available in the US, Australia, New Zealand, Canada, and the UK
  • App-based management: consumer app for managing installments

Pricing

Zip’s merchant pricing is not publicly listed and varies by market and volume. Contact Zip’s merchant team for a rate card. Account fees apply to consumers in some markets.

Where Zip shines

  • No hard credit check is the clearest differentiator for consumers concerned about credit impact
  • Multi-market availability for businesses operating across regions
  • Flexible product tiers allow higher credit limits than a standard 4-payment app

Where Zip falls short

  • Less consumer recognition in the US than Klarna, Afterpay, or Affirm
  • Account fees in some markets create friction for consumers
  • Non-transparent merchant pricing makes comparison harder

Customer reviews

Zip scores 3.8/5 on Capterra from 45 reviews and 3.7/5 on G2 from 40 reviews. Its Trustpilot score of 3.2/5 from 12,000+ reviews reflects mixed consumer experiences, particularly around account fees.

Who Zip is best for

  • Merchants serving consumers who cannot or will not accept a hard credit inquiry
  • Multi-market businesses that need one BNPL provider across several countries
  • Practices where patient accessibility at checkout is the primary concern

6. Splitit: Best affirm alternative for existing credit card installments

Splitit works differently from every other option on this list. Rather than issuing new credit, it splits the total purchase into monthly installments and charges each one to the consumer’s existing credit card. No new loan is created, no interest is added by Splitit, and no credit check is needed because no credit is issued. The consumer’s card network interest terms apply if they do not pay off their card balance monthly, but Splitit itself charges zero interest.

This model is particularly suited to high-ticket purchases where the consumer already has sufficient credit card capacity. For aesthetic clinics offering treatments in the $1,000-$5,000 range, Splitit can allow patients to self-finance without a new financing agreement.

Key features

  • No new credit issued: installments charged to existing card, not a new loan
  • Zero Splitit interest: no additional interest charged by the platform
  • No credit check: no new credit product means no inquiry
  • High-ticket suitability: suitable for purchases where existing card limit is sufficient
  • Merchant integration: available via API and major e-commerce plugins

Pricing

Splitit merchant rates are negotiated individually. The platform does not publish standard rate cards. Merchants with higher transaction volumes or specific industry verticals may negotiate better rates. Contact Splitit’s merchant team directly.

Where Splitit shines

  • No new credit issued is the most unique feature among affirm alternatives: zero regulatory complexity around new lending
  • Ideal for high-ticket purchases where consumers have card capacity
  • Zero consumer interest from Splitit itself simplifies the patient financing conversation

Where Splitit falls short

  • Requires consumer to have sufficient existing credit card limit: excludes patients who do not
  • Much lower consumer awareness than Klarna, Afterpay, or Affirm
  • Merchant network smaller than category leaders

Customer reviews

Splitit scores 4.4/5 on Capterra from 35 reviews and 4.2/5 on G2 from 30 reviews. Reviewers consistently praise the zero-interest model and suitability for high-value purchases. The most common limitation noted: “Requires existing credit card with sufficient limit.”

Who Splitit is best for

  • High-ticket retailers and aesthetic clinics where treatments cost $1,000 or more
  • Merchants who want to offer installments without issuing new credit products
  • Practices where patients are creditworthy but prefer to avoid a new loan account

Keep patient financing inside your clinical workflow

Pabau’s native Klarna integration lets aesthetic and medical clinics offer buy now, pay later at booking, without a separate checkout layer or third-party payment portal.

Pabau clinic management platform

7. Pabau: The clinic management platform for aesthetic payment plans

Pabau is not a BNPL app. It is an all-in-one practice management platform for aesthetic and medical clinics, built with a native Klarna integration that brings buy now, pay later into the clinical booking and payment workflow directly.

For aesthetic and medical clinic owners evaluating affirm alternatives, the question is not just which BNPL provider has the best rates. It is whether a retail checkout BNPL app fits inside a clinical environment, or whether something purpose-built for clinic payments is the right answer.

Pabau supports aesthetic clinics, dermatology practices, medical spas, longevity clinics, and multi-disciplinary practices across the US, UK, and beyond. Its payment processing features combine invoicing, deposits, and Klarna-powered patient financing in one system, so staff do not manage a separate BNPL checkout flow alongside their practice management software.

Key features

  • Native Klarna integration: patient financing offered at the booking stage, not via a separate checkout
  • Integrated payment processing: invoicing, deposits, and card payments inside the PMS
  • Online booking with payment collection: capture deposits or full payments at online booking
  • Digital forms and consent: pre-appointment forms integrated with the patient record
  • Automated workflows: reminder sequences, recall campaigns, and follow-up communications
  • Multi-location support: manage payment workflows across multiple clinic sites

Pricing

Plan / Tier Price Details
Starter From $65/month Per practitioner; billed monthly
Team / Group Scales by location and user count Volume-based pricing
Enterprise Custom pricing Contact sales for a quote

Where Pabau shines

  • Patient financing (Klarna) sits inside the same system as bookings, records, and communications: no separate checkout app
  • Built for clinical workflows: consent, documentation, and payments are connected to the patient record
  • Suitable for aesthetic clinics, med spas, GP clinics, dermatology, longevity, and multi-specialty practices
  • Capterra rating of 4.7/5 from 600+ verified reviews reflects strong clinic operator satisfaction

Where Pabau falls short

  • Not a standalone BNPL app: clinics that only want a payment installment tool and not a full PMS will find Pabau over-specified
  • Onboarding has a learning curve for practices migrating from simpler systems

Customer reviews

Pabau scores 4.7/5 on Capterra from 600+ verified reviews. Positive themes include comprehensive clinic management features and strong booking and scheduling tools. The most commonly noted challenge is the initial learning curve for new users.

Who Pabau is best for

  • Aesthetic clinics and med spas wanting patient financing inside their booking system, not bolted on separately
  • Multi-location practices needing unified payment and clinic management
  • Clinics that offer high-cost treatments (injectables, laser, regenerative therapies) where patient financing meaningfully improves conversion

BNPL for healthcare and aesthetic clinics: What the retail apps miss

Every affirm alternative reviewed above was built for retail e-commerce, not for clinical environments. That distinction matters more than most BNPL comparison articles acknowledge.

Three things separate clinical payment financing from retail BNPL:

  • Regulatory context: the CFPB issued an interpretive rule in 2024 clarifying that BNPL products that function like credit cards are subject to Truth in Lending Act protections under Regulation Z. Medical payment plans configured as installment credit may carry separate obligations under TILA/Regulation Z that differ from retail BNPL. Clinics should verify their specific arrangement with a compliance advisor.
  • Patient experience: retail BNPL apps prompt consumers during checkout on a website. In a clinic, the financing conversation happens with a treatment coordinator or at reception. The financing tool needs to work inside that workflow, not redirect the patient to a separate app or portal.
  • Workflow integration: Affirm, Klarna, Afterpay, and the other platforms on this list integrate with e-commerce checkouts. They do not natively connect to clinical records, booking calendars, or consent management. A clinic using one of these as a standalone tool must manage two separate systems.

Pro Tip

Check whether your BNPL provider’s merchant terms specifically permit healthcare and aesthetic services. Some providers restrict financing for certain medical categories or require additional merchant verification for clinical businesses. Verify with your chosen provider before promoting financing to patients.

Medical payment plans vs BNPL apps: Which works better for clinics?

Clinics have two broad options for offering patients flexible payment: a third-party BNPL app (Klarna, Afterpay, etc.) or an in-house medical payment plan managed through the practice management system. Each has trade-offs that depend on transaction size, patient volume, and workflow preference.

Factor Third-party BNPL app In-house medical payment plan
Setup Fast; integrate with e-commerce checkout Requires PMS with payment plan functionality
Credit risk Carried by BNPL provider Carried by the clinic
Merchant cost 3-6% + fixed fee per transaction No third-party fee; admin overhead instead
Repayment terms Fixed by provider (6 weeks to 36 months) Configurable to match treatment plan
Workflow fit Separate checkout layer outside PMS Integrated with booking and patient record
Patient experience Redirect to BNPL provider portal Handled at reception within clinic system
Regulatory complexity Managed by BNPL provider (varies by jurisdiction) Clinic may have TILA/Regulation Z obligations

For most aesthetic and medical clinics, a third-party BNPL provider integrated through a purpose-built practice management system (rather than used as a standalone checkout tool) gives the best of both options: the credit risk stays with the BNPL provider, and the workflow stays inside the clinic’s system. That is the model Pabau’s Klarna integration follows.

How to choose the best affirm alternative for your business

The right affirm alternative depends on your business type, average transaction size, patient or customer demographics, and how important workflow integration is relative to simply offering installment payments at checkout.

  • Average order value under $500: Afterpay or Sezzle’s 4-payment models cover most purchases at this level without requiring longer-term financing.
  • Average order value $500-$2,000: Klarna’s Pay in 3 or PayPal Pay Monthly cover this range. Klarna is the stronger option if you want a single provider with multiple product types.
  • Average order value above $2,000: Splitit (existing card installments) or Klarna’s 12-36 month financing products suit this range. Affirm’s 1-60 month terms also cover it if you prefer to keep Affirm.
  • Healthcare or aesthetic clinic: any of the above may work as a standalone tool, but the stronger path is a BNPL provider integrated into your practice management system. Pabau’s native Klarna integration is the only solution on this list that keeps financing inside the clinical workflow.
  • Credit check sensitivity: Zip (soft/no hard check) or Afterpay (no credit check) reduce checkout abandonment for patients cautious about credit inquiries.
  • Already on PayPal: PayPal Pay Later adds BNPL with no new vendor relationship.

For clinics that want the flexibility of Klarna’s financing products alongside a complete practice management system, integrated payment processing within a purpose-built PMS is consistently more operationally efficient than bolting a retail BNPL checkout onto a separate clinic software stack. Read about how medical spa software handles payments end-to-end for more context on where financing fits in the clinic workflow.

Conclusion

Most merchants looking for affirm alternatives are evaluating fee levels, repayment term flexibility, and credit check policies. Klarna wins on product range, Afterpay on simplicity, Splitit on zero-interest existing-card installments, and Zip on accessibility for credit-sensitive consumers.

For aesthetic and medical clinics, the bigger question is whether a retail-first BNPL app fits a clinical environment at all. Pabau’s native Klarna integration resolves that by keeping patient financing inside the booking and payment workflow, rather than requiring a separate checkout app.

Frequently asked questions

What are the best affirm alternatives with no credit check?

Afterpay and Sezzle do not perform a credit check for their standard 4-payment products. Zip offers a soft-check or no-hard-inquiry policy depending on product tier. Splitit requires no credit check because it uses the consumer’s existing credit card rather than issuing new credit. All four are practical affirm alternatives for merchants whose customers are sensitive to credit inquiries.

Which buy now pay later apps have no down payment?

Klarna (Pay in 3 and Pay in 30 days), Afterpay, Sezzle, and Zip all allow consumers to complete a purchase without an upfront down payment, spreading the full amount across installments. PayPal Pay in 4 operates the same way. Splitit places a hold on the full amount on the consumer’s credit card but charges only the first installment immediately.

Which BNPL platforms offer 12-month financing like Affirm?

Klarna offers financing from 6 to 36 months with APR. PayPal Pay Monthly covers terms up to 24 months. Affirm itself offers 1 to 60 months. Afterpay and Sezzle are limited to 6-week, 4-payment structures and do not offer 12-month financing. Splitit can extend across as many monthly installments as the card’s credit limit allows, though terms are set at the merchant level.

Can aesthetic clinics use buy now pay later for patient payments?

Yes, but the integration approach matters. Retail BNPL apps like Klarna, Afterpay, and Affirm can be used by aesthetic clinics, but they require a separate checkout flow outside the clinic’s practice management system. Pabau offers a native Klarna integration that keeps patient financing inside the booking and payment workflow, which is more operationally efficient for clinical environments. Verify that your chosen BNPL provider’s merchant terms permit aesthetic and healthcare services before promoting financing to patients.

Are there BNPL options with lower merchant fees than Affirm?

Affirm’s merchant discount rate can reach up to 30% on certain longer-term financing products, though rates vary widely by product and volume. Klarna typically charges 3-5% plus a fixed fee. Afterpay runs approximately 4-6% plus a fixed fee. Sezzle is at the higher end among short-cycle providers at around 5-6%. PayPal Pay in 4 uses standard PayPal transaction rates with no additional BNPL fee. Splitit rates are negotiated individually.

What is the best BNPL app for medical and aesthetic treatments?

For aesthetic and medical clinics, Klarna integrated through a practice management system like Pabau is the most workflow-efficient option: patient financing is offered at the booking stage without a separate checkout app. For standalone BNPL without a full PMS, Klarna’s range of repayment terms (Pay in 3 through 36-month financing) covers most aesthetic treatment price points better than the 6-week-only models offered by Afterpay and Sezzle.

How does Affirm compare to Klarna and Afterpay for merchants?

Affirm offers longer repayment terms (up to 60 months) than Klarna (up to 36 months) or Afterpay (6 weeks only). Affirm’s merchant fees can reach 30% on deferred-interest products, versus 3-5% for Klarna and 4-6% for Afterpay. Klarna offers more product variety in one integration. Afterpay is simpler but offers no long-term financing. All three are retail-first platforms not natively designed for clinical billing workflows.
