Below is the baseline for each system, and the findings that warrant escalation.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>Cardiovascular:<\/strong> Regular rate and rhythm, palpable peripheral pulses, capillary refill under two seconds, and no edema. Escalate for arrhythmia, weak pulses, pitting edema above 3+, or a new murmur.<\/li><li><strong>Respiratory:<\/strong> Bilateral breath sounds clear, unlabored breathing, no accessory muscle use, and a rate of 12 to 20. Escalate for a rigid abdomen, severe tenderness, absent bowel sounds, or distension.<\/li><li><strong>Musculoskeletal:<\/strong> Full range of motion in all joints, strength 5\/5 bilaterally, and no deformities. Escalate for limited range, weakness, joint swelling, or pain on movement. For shoulder pain, a targeted maneuver such as <a href=\"https:\/\/pabau.com\/blog\/neers-test\/\">Neer&rsquo;s test<\/a> narrows the cause.<\/li><li><strong>Skin:<\/strong> Intact, warm, dry, and an appropriate color. Check pressure points if the patient is immobile.
