from providers, checks them for errors, and forwards them to insurance payers. It also converts each claim into the format that particular payer will accept.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Practices new to insurance billing usually assume they will submit claims to each insurer one by one. The clearinghouse is the reason nobody does that. You connect once, and that single connection reaches thousands of payers.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This guide follows one claim from the invoice in your software to the payment landing back in it. It also covers what clearinghouses cost, how to pick one, and what changes when a cash-pay practice starts billing insurance.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What a clearinghouse does, step by step<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A clearinghouse takes your finished claim, checks it, translates it, and delivers it to the right payer. Here&rsquo;s the full round trip a single claim makes.<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Your billing or practice management software builds the claim from the invoice, using the same data fields as the paper <a href=\"https:\/\/pabau.com\/templates\/cms-1500-form\/\">CMS-1500 form<\/a>.<\/li>\n\n\n\n<li>The software transmits it to the clearinghouse as an 837P file. That&rsquo;s the HIPAA electronic data interchange (EDI) standard for professional claims.<\/li>\n\n\n\n<li>The clearinghouse scrubs it, checking patient details, CPT and ICD-10 codes, the payer ID, and the rendering provider&rsquo;s NPI.<\/li>\n\n\n\n<li>Anything wrong bounces back to you, usually within minutes. You fix it before the payer ever sees the claim.<\/li>\n\n\n\n<li>Clean claims route on to the payer in the format that payer accepts, batched with every other claim heading the same way.<\/li>\n\n\n\n<li>The payer acknowledges the file, then adjudicates each claim and decides what it will pay.<\/li>\n\n\n\n<li>Payment and an <a href=\"https:\/\/pabau.com\/blog\/electronic-remittance-advice\/\">electronic remittance advice<\/a>, the 835 file, travel back down the same path into your software.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">Step six has two receipts worth knowing by name. <a href=\"https:\/\/www.cms.gov\/files\/document\/mln3171902-checking-medicare-claim-status.pdf\" target=\"_blank\" rel=\"nofollow noopener\">Medicare&rsquo;s guidance on claim status<\/a> explains that a 999 acknowledgment tells you the whole file was readable. A clearinghouse collapses that into one connection, one file format, and one place to check where a claim is.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The bigger win is catching errors early. A transposed member ID or a missing modifier costs you a few minutes when the clearinghouse flags it. The same error costs you weeks if the payer finds it, because the claim has to come back, get corrected, and go out again.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The time saving is measurable. The <a href=\"https:\/\/www.dataspring.com\/advisory-services\/index-report\" target=\"_blank\" rel=\"nofollow noopener\">2024 CAQH Index<\/a> puts the average saving at seven minutes per claim when medical providers submit electronically instead of manually. That single distinction decides what you do next with the claim.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A rejection means the claim never entered the payer&rsquo;s system. Something in the data or the formatting failed a front-end check, so there&rsquo;s nothing to appeal. You correct the field and resubmit, and the claim is treated as new.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A denial means the payer read the claim, processed it, and refused to pay. The reason arrives as a <a href=\"https:\/\/pabau.com\/blog\/denial-codes-in-medical-billing\/\">claim adjustment reason code<\/a> (CARC) on the remittance. Resubmitting the same claim won&rsquo;t help. Work down this list in order.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><b>Your payers.<\/b> Search their published payer list for the specific plans you bill, not just the parent insurer.<\/li>\n\n\n\n<li><b>Native integration with your software.<\/b> If your <a href=\"https:\/\/pabau.com\/blog\/practice-management-software\/\">practice management system<\/a> can&rsquo;t talk to it, you re-key every claim by hand.<\/li>\n\n\n\n<li><b>ERA and eligibility support.<\/b> Both should be included or cheap, because you&rsquo;ll use them on every visit.<\/li>\n\n\n\n<li><b>Enrollment help.<\/b> Payer enrollment is the slowest part of going live, and some vendors do the paperwork for you.<\/li>\n\n\n\n<li><b>Support you can reach.<\/b> A rejected claim you can&rsquo;t decode is worth nothing until someone explains the code.<\/li>\n\n\n\n<li><b>Published pricing.<\/b> A vendor that won&rsquo;t put numbers on its site won&rsquo;t get simpler after you sign.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The names you&rsquo;ll meet most often, in no particular order:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><b>Claim.MD<\/b> lets you create CMS-1500 and UB-04 claims in its portal or upload batches from any billing system, and it publishes its prices.<\/li>\n\n\n\n<li><b>Availity<\/b> runs a national multi-payer network and portal, and several large insurers route their provider transactions through it.<\/li>\n\n\n\n<li><b>Office Ally<\/b> says more than 80,000 healthcare organizations use its software and clearinghouse, and its base clearinghouse tier starts free.<\/li>\n\n\n\n<li><b>Waystar<\/b> is a full <a href=\"https:\/\/pabau.com\/blog\/what-is-revenue-cycle-management\/\">revenue cycle<\/a> platform, covering eligibility, claims, denials, and payments rather than submission alone.<\/li>\n\n\n\n<li><b>TriZetto Provider Solutions<\/b>, part of Cognizant, is an enterprise clearinghouse and revenue cycle service aimed at larger groups and health systems.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Of everything on that list, integration decides the most. A clearinghouse your software can&rsquo;t reach means someone types every claim twice, once into your system and once into a portal.
