Medicare denies it every time, and the reason has nothing to do with your diagnosis or your modifier.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">LCD L33784 rules out the water circulating system itself, and it names the replacement pad in the same sentence. So the money on this line turns on the conversation before delivery, not on the claim form.<\/p>\n\n\n\n<h2 id=\"h-e0249-covers-the-replacement-pad-and-nothing-else\" class=\"wp-block-heading\">E0249 covers the replacement pad, and nothing else<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The official descriptor for E0249 is \u00ab\u00a0pad for water circulating heat unit, for replacement only\u00a0\u00bb. Two words carry the limit, replacement only.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The pad itself is flexible and holds a series of channels. Water heats in an external reservoir, and an electric pump pushes it through those channels against the patient&rsquo;s skin.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Policy article A52502 defines the whole system under code E0217, which already includes the pump, the pad, and every accessory the pad needs to work.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">So E0249 has no place on an initial supply. Both alternatives lose on that comparison alone.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The policy calls a water circulating heating pad system <em>\u00ab\u00a0not medically necessary\u00a0\u00bb<\/em>. On that basis, <em>\u00ab\u00a0a replacement pump (E0236) or pad (E0249, A9999) will be denied as not reasonable and necessary.\u00a0\u00bb<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Your pad is named there, by code. There is no indication list to satisfy and no qualifying diagnosis to find, because Medicare settled the question at the level of the system.<\/p>\n\n\n\n<h3 id=\"h-the-replacement-pad-cannot-outrank-the-system\" class=\"wp-block-heading\">The replacement pad cannot outrank the system<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Parts inherit the status of the equipment they belong to. Medicare will pay to keep a covered item running, and it will not fund upkeep on an item it never accepted.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Two objections come up at the front desk. Here is what each one is worth.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>\u00ab\u00a0The patient bought the unit himself, so the pad is a separate purchase.\u00a0\u00bb<\/strong> Buying the system privately does not create coverage for its parts.<\/li>\n\n\n\n<li><strong>\u00ab\u00a0The pad has passed its reasonable useful lifetime.\u00a0\u00bb<\/strong> That rule decides when Medicare replaces equipment it already paid for. Medicare paid nothing toward this system, so the clock never started.<\/li>\n<\/ul>\n\n\n\n<h3 id=\"h-e0210-is-the-pad-medicare-does-cover\" class=\"wp-block-heading\">E0210 is the pad Medicare does cover<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">One code in this family carries genuine coverage. The policy calls a standard electric heating pad <em>\u00ab\u00a0reasonable and necessary\u00a0\u00bb<\/em> for four purposes:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>To <em>\u00ab\u00a0relieve certain types of pain\u00a0\u00bb<\/em>.<\/li>\n\n\n\n<li>To <em>\u00ab\u00a0decrease joint and soft tissue stiffness\u00a0\u00bb<\/em>.<\/li>\n\n\n\n<li>To <em>\u00ab\u00a0relax muscles\u00a0\u00bb<\/em>.<\/li>\n\n\n\n<li>To <em>\u00ab\u00a0reduce inflammation\u00a0\u00bb<\/em>.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">E0210 has its own product definition to meet. The pad needs a fabric cover, a timing device for automatic shut-off, and certification by Underwriters Laboratories.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">One patient group is ruled out by name. The policy treats a heating pad as not reasonable and necessary for pain from peripheral neuropathy, diabetic neuropathy included.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That gives your front desk a straight answer. A <a href=\"https:\/\/pabau.com\/industry\/chiropractic-software\/\">chiropractic practice<\/a> sending a patient home with heat can bill E0210 and expect payment. Reach for the water circulating version and the claim dies, as does the moist pad under E0215.<\/p>\n\n\n\n<h2 id=\"h-e0249-or-a9999-the-pad-s-material-decides-the-code\" class=\"wp-block-heading\">E0249 or A9999: The pad&rsquo;s material decides the code<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Before you pick a code, the pad has a product test to pass. The coding guidelines in <a href=\"https:\/\/www.cms.gov\/medicare-coverage-database\/view\/article.aspx?articleid=52502\" target=\"_blank\" rel=\"nofollow noopener\">policy article A52502<\/a> set it out, and it turns on what the pad is made of.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">E0249 is the durable pad for an E0217 system. It is made of rubber, heavy plastic, or durable fabric, it can be cleaned, and it is designed for long term use.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A replacement pad made of other material, built for shorter term use, must go on A9999 instead.
