That might be scar tissue, restricted joint movement, or a chronic nail deformity. Active wound care stops. The sequela begins.

The ICD-10-CM Official Guidelines add a second rule, set out in Section I.C.19.a. When S is used, you also assign a code for the specific sequela condition being treated. S61.311S is the causal code, so it sits second, behind the sequela diagnosis that brought the patient in.

Reversed sequencing is the next most common failure after picking the wrong 7th character. Many billing systems flag it automatically. The harder problem sits upstream, where the sequela condition may never have been documented at all. The same rule applies to every injury sequela, including S33.8XXS.

S61.311A vs. S61.311D vs. S61.311S: Choosing the right variant

The split between D and S trips up even experienced coders. The test is whether active treatment is still ongoing.

Still doing wound care, dressing changes, or suture removal? Use D for a subsequent encounter.

Wound healed, but the patient is back for scar management, nail reconstruction, or stiffness? Use S for sequela.

First visit for this injury, anywhere? Use A. Even if the patient was treated elsewhere first, A applies to the first encounter at your facility.

A repair that is still at the suture-removal stage, billed under CPT code 15851, sits firmly in D territory. Once that visit is done and the chart says so, the door to S opens. The same pattern shows up in S01.111D, where a laceration is still under active care.

Clinical description: what each qualifier means

Each word in the S61.311S descriptor encodes a specific clinical finding. Knowing the anatomy behind the code helps coders catch missing detail before claims go out.

Laceration: A jagged or irregular wound from a sharp object or a tearing force. It is distinct from an incision, a puncture, or an abrasion. The mechanism has to be documented as a laceration, not assumed. A puncture wound is coded elsewhere in the S61 block, such as S61.238A.

Without foreign body: No retained material sits in the wound at the time of the encounter. Glass, metal, and wood splinters all count. If a foreign body came out at an earlier visit and none remains, this qualifier still holds.

Left index finger: Laterality is mandatory, and the index finger is the second digit. Left specificity rules out S61.310S, the right index finger equivalent. Without explicit left or right documentation, the claim cannot go out as S61.311S.

With damage to nail: The nail plate, nail bed, or surrounding tissue shows clinically significant damage. Nail involvement is what puts the injury in S61.3 rather than S61.2. The left index finger equivalent without nail damage is S61.211S. That detail has to appear in the clinical note, not only in a procedure summary.

Sequela: The encounter happens after active treatment has concluded. The presenting complaint is a late effect of the original laceration.

Practices managing wound care and post-injury follow-up lean on structured clinical record keeping. If the wound is healed and the patient keeps returning for scar or nail sequelae, those encounters may need S61.311S rather than the D character. Continued use of D past the active treatment phase is one of the most common triggers for payer audits on finger laceration claims.

Common coding errors and how to prevent them

A few failure patterns recur across practices billing S61.311S and its siblings. Each one is avoidable with a clear documentation protocol.

Error 1: Using D when S is correct

This is the most common one. Coders keep billing S61.311D for months after the wound has healed, because the chart notes stop mentioning the injury. When the patient returns for nail reconstruction or scar management, a new note goes in without re-establishing the encounter type.

The result is a D code trail for a condition that is already in the sequela phase. The fix is to build an explicit « active treatment concluded » entry into the final wound-care note. That gives billing a clear transition point and supports the move to S coding.

Error 2: Missing the sequela condition code

S61.311S cannot stand alone as the primary diagnosis. ICD-10-CM guidelines require a code for the sequela condition, such as scar contracture, chronic pain, or nail deformity, to be listed first. Submitting S61.311S on its own will draw a return-to-provider request or a denial from most payers.

Error 3: Omitting nail involvement

Nail damage documented at the initial visit sometimes drops out of later notes, and coders default to the without-nail variant. S61.211S and S61.311S sit in different ICD-10-CM categories.

Billing the without-nail version when the original injury damaged the nail misrepresents the clinical history. The same nail-damage split runs through neighboring finger categories, including S60.159D.

Error 4: Incorrect laterality

Laterality errors cluster where billing teams abstract from scanned paper notes or short dictations. S61.310S and S61.311S generate different claims. Payers can cross-reference the original encounter claim for laterality, and a mismatch flags the account for review. Claims management software with laterality validation catches this before submission.

Pabau’s claims tracking shows where every S61.311S claim sits, so a laterality or encounter-type rejection surfaces in days rather than weeks.

Approximate synonyms and alphabetic index paths

The ICD-10-CM alphabetic index reaches S61.311S through several entry points. By the time billing opens the chart, the sequela condition note is missing, laterality is vague, and nobody recorded the encounter-type transition.

Practice management software like Pabau holds laterality, wound description, and encounter phase as structured fields, not free text. When a clinician documents a finger laceration with nail damage at the first visit, that detail travels with the record. At the sequela encounter, the note template surfaces the original injury automatically.

So providers confirm or update the encounter status without rebuilding the history from memory. Embedding the ICD-10 lookup inside the clinical workflow, rather than in a separate tab, cuts the context switching that causes encounter-type errors.

Practices handling wound care, nail deformity, and scar management get the same benefit from Pabau’s dermatology EMR software for tracking injuries over time.

Reduce ICD-10 coding errors at the visit

Pabau’s structured clinical records capture laterality, wound description, and encounter type before billing reviews the chart. S61.311S packs five clinical qualifiers into one code, and each has to match the record. The most preventable failure is the D-to-S handoff that never happens.

So make that transition explicit. One line in the final wound-care note, saying active treatment has concluded, is what lets billing move to S coding without guessing. It costs a clinician a few seconds and prevents a denial.

Structured records make that habit easy to keep, because the encounter phase is a field rather than a sentence someone remembers to write. Book a demo to see how Pabau captures laterality and encounter type at the visit.

What does ICD-10 Code S61.311S mean?

S61.311S is a billable ICD-10-CM code for laceration without foreign body of the left index finger with damage to nail, sequela. It applies once active wound care has ended and a late effect still needs treatment. The code is valid for FY2026 claims.

When should you use the sequela (S) 7th character in ICD-10?

Use S when active treatment for the original injury has ended and the patient returns for a late effect. Scar tissue, joint stiffness, and chronic nail deformity all qualify. The guidelines require the sequela condition to be the primary diagnosis, with S61.311S as an additional code.

What is the difference between S61.311A, S61.311D, and S61.311S?

S61.311A is the initial encounter, used the first time a patient gets active treatment for the laceration. S61.311D covers follow-up visits while active treatment continues. S61.311S is the sequela code, used only after treatment has ended and a late effect persists. Using D when S applies is a leading cause of denials.

Is S61.311S a billable ICD-10-CM code?

Yes. S61.311S is billable and specific, valid for FY2026 claims. It works on physician claims, facility claims, and anywhere a specific ICD-10-CM code is required. The documentation has to support all five qualifiers: laceration, no foreign body, left index finger, nail damage, and sequela.

What documentation is required to use S61.311S?

The record must name the left side and the index finger, describe a laceration, and confirm no retained foreign body. It also has to document damage to the nail or nail bed. Finally, it has to show that active treatment ended and this visit addresses a sequela. The sequela diagnosis itself is coded separately and listed first.

What ICD-10 code is used for laceration of the left index finger with nail damage?

It depends on the encounter. S61.311A covers the initial encounter, S61.311D covers subsequent encounters during active treatment, and S61.311S covers sequela encounters. All three describe laceration without foreign body of the left index finger with damage to nail.
