A Type 2 NPI belongs to the organization that bills for the care. That one distinction decides which number goes in which field on every claim you send.

Under HIPAA, every covered healthcare provider needs a National Provider Identifier before submitting an electronic transaction. Which one you need depends on how your practice is structured. Employed clinicians usually need only a Type 1. Anything that bills under a business name and its own EIN needs a Type 2 as well.

Submitting claims with the wrong number is a common source of preventable rejections. This guide covers what each NPI is, who needs which, and when you need both. It also shows where each number lands on the CMS-1500 claim form.

Type 1 vs Type 2 NPI: Key differences at a glance

The NPI system has exactly two enumeration types, and they serve different purposes. One follows the clinician. The other follows the business. Sorting this out before you apply saves weeks of correction work with payers.

Attribute

Type 1 NPI (individual)

Type 2 NPI (organizational)

Issued to

Individual clinicians and sole proprietors

Organizations, group practices, hospitals, and facilities

Tied to

The individual, and it persists across employers

The business entity and its EIN

Changes when you change jobs?

No, it stays with you permanently

Not applicable, the number belongs to the entity

Application identifier

Social Security Number (SSN) or Individual Taxpayer Identification Number

Employer Identification Number (EIN)

Used on claims as

Rendering or attending provider NPI

Billing provider NPI

Claim form field

Box 24J on CMS-1500

Box 33a on CMS-1500

Both are issued through the NPPES portal, administered by the Centers for Medicare and Medicaid Services (CMS). Applications are free. Online applications typically take up to 10 business days to process, and CMS emails the number once the review is finished. Paper applications take about 20 business days.

What is a Type 1 NPI?

A Type 1 NPI is assigned to an individual healthcare provider rather than to a business. Think of it as a professional ID number that belongs to you for life. It does not change when you join a new practice, move states, or return to clinical work after a break.

Individual providers who need a Type 1 NPI include:

Physicians, nurse practitioners, and physician assistants

Dentists, optometrists, and pharmacists

Chiropractors, physical therapists, and occupational therapists

Psychologists and licensed clinical social workers

In short, any clinician who delivers care and takes part in electronic transactions with payers needs one. Because the number travels with the person, your HIPAA compliance records should track it per clinician rather than per location.

Sole proprietors are individual providers too. They may still need a Type 2 NPI if they bill through a separate business entity. More on that below.

Who needs a Type 1 NPI?

Any individual who is a HIPAA-covered provider and takes part in an electronic health transaction needs a Type 1 NPI. For most licensed healthcare professionals in the US, this is not optional.

The eligibility list is broad. The public NPI registry run by CMS shows how wide that group is. It covers clinicians who bill insurance, receive Medicare or Medicaid reimbursement, or send any electronic administrative transaction to a payer.

Can you apply before you are licensed? NPPES accepts applications before licensure in some circumstances, but payers will not credential you until your license is confirmed. If you are starting a medical practice, apply early. Credentialing is usually the longest bottleneck in a launch.

What is a Type 2 NPI (organizational NPI)?

A Type 2 NPI is issued to an organization that provides healthcare services or bills payers as a business. It is tied to an EIN rather than to an individual’s SSN. The organization keeps the same number even after every member of staff has turned over.

Organizations that require a Type 2 NPI include:

Group medical practices with two or more providers

Hospitals and health systems

Outpatient clinics and ambulatory surgery centers

Home health agencies and hospice organizations

Durable medical equipment (DME) suppliers

Pharmacies operating as a business entity

Med spas and aesthetic practices billing under a business name

Mental health group practices

The test is whether care is billed under a business entity’s name and EIN. If it is, that entity needs its own Type 2 NPI. Practices that outsource administration to a management services organization still bill under the clinical entity, so the Type 2 NPI stays with that entity.

For multi-specialty practices, storing both numbers correctly is what keeps claims moving. Check that your HIPAA compliance software holds a field for the organizational number as well as one per clinician.

Who needs a Type 2 NPI?

Any healthcare organization that bills payers under its own name needs a Type 2 NPI. Size is irrelevant. A two-provider dermatology group, a single-location med spa billing under an LLC, and an outpatient mental health practice all need one.

The billing test is simple. If the name in the claim form’s billing provider field belongs to a business rather than a clinician, that business needs a Type 2 NPI.

This is one of the most frequently misconfigured settings when practices first set up their claims management software. Putting a Type 1 number in the billing provider field when a Type 2 is required causes rejections that take weeks to clear.

Subpart NPIs for multi-location organizations

A single legal entity can hold more than one Type 2 NPI. CMS calls the extra numbers subparts, and most comparison guides never mention them.

A subpart is a component of an organization that bills payers separately. A hospital rehabilitation unit or a second site with its own payer contracts can qualify. Each subpart gets its own Type 2 NPI under the parent organization’s EIN. Decide subpart status before you enroll, because payers build their contracts around the number you hand them.

Pabau’s claims tools pull the right Type 1 and Type 2 NPI into each claim, so billing leaves clean.

Do you need both a Type 1 and Type 2 NPI?

Some providers need both. The requirement usually surfaces when a first batch of claims comes back rejected. The answer depends on how your practice is legally structured.

You need both if:

You are a sole proprietor billing through a separately registered entity, such as an LLC, PLLC, or PC with its own EIN

You are an individual clinician who also owns or operates a group practice

You render services in several settings, some billed under your individual number and some under an organizational one

You likely need only a Type 1 NPI if:

You are an employed clinician billing entirely through your employer’s organizational NPI

You are a sole proprietor who bills insurance under your own name rather than a business entity

The sole proprietor case trips up many providers. If you practice as « Jane Smith, NP » with no separate entity, your Type 1 NPI goes in both the rendering and billing provider fields.

If you formed « Smith Wellness PLLC » and obtained an EIN for it, that entity needs its own Type 2 NPI. Both numbers then sit on different lines of the same claim.

Pro Tip

Before your first claim submission, check whether your practice bills under an individual name or a business name. If the biller is an entity with its own EIN, that entity needs a Type 2 NPI on file before any electronic claims go out.

How to apply for an NPI number through NPPES

Both numbers come from the same system, the NPPES portal managed by CMS. The application is free and takes most providers 15 to 20 minutes online. Here is the process:

Create an NPPES account at nppes.cms.hhs.gov using a valid email address. Anyone applying on behalf of an organization needs their own NPPES credentials.

Select your entity type. Choose Entity Type 1 for an individual or Entity Type 2 for an organization. This is the most consequential selection in the form.

Enter identifying information. Type 1 applicants give an SSN or Individual Taxpayer Identification Number, and Type 2 applicants give the organization’s EIN. The SSN is used only for identity verification and never appears in the public registry.

Provide professional information. This covers license numbers, state of licensure, contact details, and the taxonomy codes that describe the services you render.

Submit the application. Online submissions typically take up to 10 business days to process. CMS emails your number once the review is complete, so nothing is assigned instantly. A paper application takes about 20 business days.

Once your number is assigned, add it to your practice management system and give it to every payer during credentialing. Claims still get rejected after a valid NPI is issued when one system is left holding the old details. Building paperless and HIPAA-compliant workflows from day one keeps that from happening.

Using NPI numbers in insurance billing and claims

Where each number appears on a claim form is set by rule. The CMS NPI standards define which fields each one occupies on the CMS-1500 and the UB-04. Outpatient providers bill on the CMS-1500, while hospitals and facilities use the UB-04.

On the CMS-1500, the rendering or attending provider’s Type 1 NPI goes in Box 24J. The billing provider’s number goes in Box 33a. For an organization, that is the Type 2. A solo practitioner billing under their own name uses their Type 1 in both fields.

Payers read those two fields together to check credentialing for the clinician who delivered care and for the entity submitting the claim. A mismatch with what the payer holds on file triggers an automatic rejection.

A few common billing situations are worth walking through:

Group practice employee: The rendering provider’s Type 1 NPI goes in Box 24J and the group’s Type 2 NPI goes in Box 33a.

Solo practitioner with no business entity: The provider’s Type 1 NPI appears in both Box 24J and Box 33a.

Solo practitioner with an LLC: The provider’s Type 1 NPI goes in Box 24J and the LLC’s Type 2 NPI goes in Box 33a.

Payers also check that the codes on a claim fit the taxonomy attached to the rendering provider’s Type 1 NPI. A surgical line such as CPT code 15822 should sit under a surgical specialty. A behavioral health line billed with HCPCS code H2031 should sit under a mental health taxonomy, with the group’s Type 2 NPI in Box 33a.

Configuring this inside your EHR and billing integration is what keeps the two numbers straight. When NPI data lives in one system and claims leave from another, someone retypes it and errors follow.

How Pabau helps practices manage NPI numbers and billing

Most practices end up holding NPI numbers in two places, the billing software and someone’s spreadsheet. Whoever builds the claim picks a number by hand, and that is where the wrong one gets used.

Practice management software like Pabau stores both Type 1 and Type 2 numbers inside the platform. Each clinician’s profile holds their individual Type 1. The practice or billing entity profile holds the organizational Type 2.

When Pabau builds an electronic claim, it applies the correct number to the correct field based on how the billing entity is set up. Nobody has to remember which one belongs where.

For multi-location practices or groups with several billing entities, this matters more. Each location can carry its own Type 2 number while a provider keeps one Type 1 across all of them.

Pabau then pulls the right pair based on where the service happened and which entity billed for it. The wider overview of practice management software shows where billing sits in that workflow.

The same setup covers med spas, where owners often treat patients themselves and bill through their own entity. Getting both numbers configured on day one saves months of credentialing and claim clean-up later.

Stop tracking NPI numbers in a spreadsheet

Pabau stores both Type 1 and Type 2 NPIs in provider profiles and applies the correct number automatically on every electronic claim. class="schema-faq-section" id="faq-question-1786435434831">
What is the difference between a Type 1 and Type 2 NPI?

A Type 1 NPI is issued to an individual healthcare provider and stays with that person for life, wherever they work. A Type 2 NPI is issued to an organization that bills insurance under a single entity name and EIN. The Type 1 vs Type 2 NPI distinction decides which number appears in which field on the CMS-1500 claim form.

Who needs a Type 2 NPI?

Any organization that bills healthcare services under its own business name needs a Type 2 NPI. That includes group practices, hospitals, outpatient clinics, DME suppliers, and med spas operating as an LLC or other legal entity. If the billing provider on a claim is a business rather than an individual, that business needs a Type 2 NPI.

Do I need both a Type 1 and Type 2 NPI?

You need both if you are an individual clinician who also owns or operates a separate business entity that bills insurance. A nurse practitioner who bills some care under their own name and other care through their PLLC needs both. Providers who bill entirely through an employer’s organizational NPI typically need only a Type 1.

How long does it take to get an NPI number?

Online applications through NPPES typically take up to 10 business days to process. CMS emails the number once the application clears review, so nothing is issued instantly. A paper application takes about 20 business days.

Can I apply for an NPI without a license?

NPPES accepts applications in some circumstances before licensure is complete. Payers and insurers will not credential or reimburse you until your license is confirmed. Applying early during the licensing process is still worthwhile, because it takes NPI processing time off the critical path for a practice launch.

What happens to my NPI if I change employers?

Your Type 1 NPI stays with you permanently. It does not change when you switch employers, move states, or change your practice setting. You update your NPPES record with the new practice address and taxonomy codes, but the 10-digit number itself stays the same for your whole career.

How do I look up my NPI number?

Search the public NPI registry at npiregistry.cms.hhs.gov by name, number, or location.
