Alan Jette and colleagues built it in 1986 and <a href=\"https:\/\/www.sralab.org\/rehabilitation-measures\/functional-status-questionnaire\" target=\"_blank\" rel=\"nofollow noopener\">validated it in primary care<\/a>. It is now used across rehabilitation and home-based care programs too.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A symptom checklist or a <a href=\"https:\/\/pabau.com\/templates\/normal-review-of-systems\/\">review of systems<\/a> captures disease severity. Administration takes 10 to 15 minutes, which keeps it practical in a busy outpatient schedule.<\/p>\n\n\n\n<h2 id=\"h-how-to-administer-and-score-it\" class=\"wp-block-heading\">How to administer and score it<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Five steps take the questionnaire from intake to a documented score. Nothing here needs new staff or new training. Ask them to answer for a typical week. If vision, literacy, or cognition gets in the way, read the items aloud and let them choose. Note that accommodation in the record.<\/li>\n\n\n\n<li><strong>Average the responses in each subscale.<\/strong> The response scale varies by section, running 0 to 4, 1 to 4, or 1 to 6. Sum the item values within a subscale, then divide by the highest possible total. Multiply by 100. A physical ADL subscale summing to 24 out of 36 gives a score of 67.<\/li>\n\n\n\n<li><strong>Read the scores in clinical context.<\/strong> Higher scores mean better function. Anything at 70 or above suggests good independent function. A score of 40 to 69 points to moderate limitation that needs targeted support. Below 40 means significant dependency. Compare against any earlier FSQ to see which direction the patient is moving.<\/li>\n\n\n\n<li><strong>Document the results.<\/strong> Record all six subscale scores, the date, and any change to how the form was administered. Add your clinical impression of function against the treatment goals. That gives you an audit-ready record and a clear starting point at the next visit.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">Digital administration compresses all five steps. The patient completes the form at check-in, the system calculates the subscales, and the results drop into the clinical note. Manual arithmetic disappears, and so does the transcription error that comes with it.<\/p>\n\n\n\n<h2 id=\"h-who-the-questionnaire-is-for\" class=\"wp-block-heading\">Who the questionnaire is for<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Any outpatient practice, hospital clinic, or home health service that needs a functional baseline can use the FSQ.
