Two weeks later, nobody can say who was supposed to order the blood test.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The meeting was fine, but the record of it was not. A multidisciplinary review only changes care when the decisions leave the room attached to a name and a date.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The template above covers the five sections a usable record needs. What follows is how experienced teams work through them, and where reviews come unstuck.<\/p>\n\n\n\n<h2 id=\"h-what-a-multidisciplinary-review-decides\" class=\"wp-block-heading\">What a multidisciplinary review decides<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A multidisciplinary review is a single scheduled meeting where clinicians from different specialties agree one plan for one patient. Physicians, nurses, pharmacists, and therapists each bring their own read of the case. Most teams shorten the name to MDT, for multidisciplinary team.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In practice, the review does three things:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Reconciles treatment plans that were written separately, by different people<\/li>\n\n\n\n<li>Settles disagreements while everyone is still in the room<\/li>\n\n\n\n<li>Gives the patient and the notes one version of what happens next<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Done well, it shows up in numbers a practice can measure: duplicate tests, clashing prescriptions, and repeat appointments all drop. The review is also where a care plan gets its authority. A typical team looks like this:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Medical lead.<\/strong> The clinician responsible for overall care, and usually the person who signs off the plan.<\/li>\n\n\n\n<li><strong>Nurses.<\/strong> Clinical and specialist nurses who see the patient most often and notice what is changing.<\/li>\n\n\n\n<li><strong>Pharmacist.<\/strong> Owns the medication list, the interactions, and the case for stopping a drug.<\/li>\n\n\n\n<li><strong>Allied health.<\/strong> Physical therapy, <a href=\"https:\/\/pabau.com\/industry\/occupational-therapy-software\/\">occupational therapy<\/a>, and speech therapy, depending on what the patient is working on.<\/li>\n\n\n\n<li><strong>Social work or care coordination.<\/strong> Housing, transport, and funding, the practical barriers that quietly sink a plan.<\/li>\n\n\n\n<li><strong>The patient.<\/strong> Increasingly present for part of the meeting, so preferences are heard before decisions are made.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Three or four people who came prepared beat eight who are reading the notes for the first time. Invite the ones whose input changes the plan, and send everyone else the summary.<\/p>\n\n\n\n<h2 id=\"h-the-five-sections-a-usable-record-needs\" class=\"wp-block-heading\">The five sections a usable record needs<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Five sections do the work. Nobody should be reading out a medical record number while five people wait.<\/p>\n\n\n\n<h2 id=\"h-how-the-review-runs-minute-by-minute\" class=\"wp-block-heading\">How the review runs, minute by minute<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A focused review of one complex patient takes about 20 minutes. Here is where that time goes:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li><strong>The day before.<\/strong> Circulate the case summary, the current medication list, and any <a href=\"https:\/\/pabau.com\/templates\/comprehensive-assessment\/\">comprehensive assessment<\/a> done since the last review. Reviews drift when nobody set a question.<\/li>\n\n\n\n<li><strong>Two minutes: framing.<\/strong> The medical lead states why this patient, why now, and what has to be decided today.<\/li>\n\n\n\n<li><strong>Ten minutes: specialist input.<\/strong> Each contributor gets a slot and sticks to findings, concerns, and what they would change.<\/li>\n\n\n\n<li><strong>Five minutes: decisions.<\/strong> The chair works through the disagreements out loud and lands each one: continue, change, or stop.<\/li>\n\n\n\n<li><strong>Three minutes: actions.<\/strong> Every decision gets an owner, a date, and a way of telling whether it worked.<\/li>\n\n\n\n<li><strong>Before anyone leaves.<\/strong> Agree the next review date, and the trigger that pulls it forward early.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">Then get the summary out within a week, to the patient and to everyone who was in the room. It takes five minutes and saves ten.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The case summary and current medication list have been circulated, not promised<\/li>\n\n\n\n<li>The question the team has to answer is written down in one line<\/li>\n\n\n\n<li>Consent to discuss the case is recorded, or the reason it isn&#8217;t needed<\/li>\n\n\n\n<li>Results the team will ask about are in the record, not in someone&#8217;s inbox<\/li>\n\n\n\n<li>One person is named as scribe, and it isn&#8217;t the person chairing<\/li>\n\n\n\n<li>The form is open on screen before the first person speaks<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The scribe point matters more than it sounds. The pharmacist leads the assessment instead of the medical lead.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">More than 40% of US adults aged 65 and over take five or more prescription drugs, according to <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10491561\/\" target=\"_blank\" rel=\"nofollow noopener\">published research<\/a>. At that point, interactions and duplicate therapy become likely rather than possible.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A good pharmacist arrives with the list already worked through:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Every drug, with its indication, dose, and how long it has been running<\/li>\n\n\n\n<li>Duplications, and the interactions that matter at this patient&#8217;s kidney function<\/li>\n\n\n\n<li>Drugs that are questionable for this age, condition, or combination<\/li>\n\n\n\n<li>Candidates for deprescribing, ranked by what is safest to stop first<\/li>\n\n\n\n<li>Adherence problems, and the reason behind them<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The team then decides continue, change, or stop for each drug. Write the reason next to the decision. The clinician who reverses it in six months needs to know what you were weighing.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Send the patient home with an updated <a href=\"https:\/\/pabau.com\/templates\/medication-schedule\/\">medication schedule<\/a>, not a verbal summary. In <a href=\"https:\/\/pabau.com\/industry\/mental-health-emr\/\">mental health practices<\/a>, the same review covers side effects and blood monitoring. It also covers whether the patient is willing to keep taking the drug at all.<\/p>\n\n\n\n<h2 id=\"h-how-to-fill-it-in-without-slowing-the-meeting-down\" class=\"wp-block-heading\">How to fill it in without slowing the meeting down<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Complete the form during the meeting, not after it. Notes written from memory that evening lose the detail that made the decision defensible.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Pre-fill what doesn&#8217;t need discussing.<\/strong> Demographics, the problem list, and current medications can come from your <a href=\"https:\/\/pabau.com\/features\/patient-intake-software\/\">intake forms<\/a> the day before.<\/li>\n\n\n\n<li><strong>Write decisions the way you would say them.<\/strong> \u00abStopped metformin, declining renal function.
