So the conversation you have before the first stroke matters as much as your technique, and a consent form is where that conversation gets written down.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Plenty of brow studios work from a form they picked up during training. It lists a few risks, collects a signature, and stops there, but six weeks later, when pigment heals patchy and the client is upset, that page has to hold up.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">So it pays to know exactly what belongs on the page, and what your state expects you to say on it.<\/p>\n\n\n\n<h2 id=\"h-what-a-microblading-consent-form-proves\" class=\"wp-block-heading\">What a microblading consent form proves<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A microblading consent form proves three things:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>The client knew what the procedure involves.<\/li>\n\n\n\n<li>They were told what can go wrong. <\/li>\n\n\n\n<li>They chose to go ahead anyway.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">Microblading is a semi-permanent tattoo, so the client is agreeing to something they cannot wash off. Insurers, licensing officers and courts all read the form as evidence of what you said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Healing takes four to eight weeks, results shift with skin type, and a touch-up is usually needed, so someone who reads that up front is far less likely to feel misled later.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Inspections are the other reason to keep the paperwork tidy. Local health departments in the US and environmental health officers in the UK both expect signed consent on file for every client.<\/p>\n\n\n\n<h2 id=\"h-what-to-put-on-the-form-section-by-section\" class=\"wp-block-heading\">What to put on the form, section by section<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A form that holds up covers seven areas:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Client details and contact information<\/li>\n\n\n\n<li>Medical history and contraindications<\/li>\n\n\n\n<li>Risks and side effects, named individually<\/li>\n\n\n\n<li>The consent statement itself<\/li>\n\n\n\n<li>An aftercare agreement<\/li>\n\n\n\n<li>Photography permission, kept optional<\/li>\n\n\n\n<li>Client and technician signatures, each dated<\/li>\n<\/ol>\n\n\n\n<h3 id=\"h-start-with-details-you-can-act-on-later\" class=\"wp-block-heading\">Start with details you can act on later<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The top of the page records who the client is and how to reach them. The downloadable template asks for the following:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Full name<\/li>\n\n\n\n<li>Date of birth<\/li>\n\n\n\n<li>Age<\/li>\n\n\n\n<li>Gender<\/li>\n\n\n\n<li>Address<\/li>\n\n\n\n<li>Contact number<\/li>\n\n\n\n<li>Email address<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Date of birth does more work than it looks. It documents that the client is old enough for body art, which for tattooing means 18 or over in most states.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Two fields are worth adding by hand. An emergency contact helps if something happens mid-appointment. The appointment date, written next to the signature, stops any argument about when consent was given.<\/p>\n\n\n\n<h3 id=\"h-screen-the-medical-history-before-you-pick-up-a-blade\" class=\"wp-block-heading\">Screen the medical history before you pick up a blade<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The template keeps this part short. Four yes or no questions cover the ground that matters most:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Do you have any allergies, and if so, which ones?<\/li>\n\n\n\n<li>Are you pregnant or breastfeeding?<\/li>\n\n\n\n<li>Do you have any skin conditions, such as eczema or psoriasis?<\/li>\n\n\n\n<li>Are you taking any medications or supplements?<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Treat every \u00abyes\u00bb as a prompt to ask a follow-up out loud. Write the answer on the form while the client is still in front of you.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Some conditions need more than a check mark. Blood thinners raise bleeding risk, poorly controlled diabetes slows healing, and a keloid history changes the scarring conversation. To screen for those, add extra lines or pair the consent with a fuller <a href=\"https:\/\/pabau.com\/templates\/med-spa-consultation-form-template\/\">med spa consultation form<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Pregnancy is the answer most technicians treat as an automatic defer. For anything else that gives you pause, ask for a <a href=\"https:\/\/pabau.com\/templates\/diagnosis-letter-from-doctor\/\">letter from their doctor<\/a> before you confirm the booking.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Where a client lists several prescriptions, ask them to bring a written <a href=\"https:\/\/pabau.com\/templates\/medication-schedule\/\">medication schedule<\/a>. It is faster than working through a phone gallery of pill packets at the door.<\/p>\n\n\n\n<h3 id=\"h-list-the-risks-in-plain-language\" class=\"wp-block-heading\">List the risks in plain language<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Write the risks the way you would say them to someone&#8217;s face. The template names five:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Temporary redness or swelling<\/li>\n\n\n\n<li>Infection where aftercare is skipped<\/li>\n\n\n\n<li>Allergic reaction to pigments or tools<\/li>\n\n\n\n<li>Scarring, granulomas or keloids<\/li>\n\n\n\n<li>Uneven pigment retention or migration<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">Three more are worth writing in. Asymmetry, a color that heals warmer than expected, and the cost of correction are the outcomes clients tend to raise afterwards.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Then comes the consent statement, the part that carries the legal weight. In the template, the client confirms that they:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Disclosed all relevant medical history and allergies<\/li>\n\n\n\n<li>Understand the procedure, its risks and the expected result<\/li>\n\n\n\n<li>Agree to follow the aftercare their technician provides<\/li>\n\n\n\n<li>Accept that results vary with skin type and healing<\/li>\n<\/ul>\n\n\n\n<h3 id=\"h-tie-aftercare-to-the-signature-then-hand-it-over-separately\" class=\"wp-block-heading\">Tie aftercare to the signature, then hand it over separately<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The consent form needs to record that the client agreed to follow aftercare. It does not have to contain the aftercare itself.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That is how the template handles it, with a single line committing the client to the instructions you provide. Keeping the instructions on their own sheet makes them easier to update, and easier for the client to stick on the fridge.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Hand that sheet over before they leave. A short <a href=\"https:\/\/pabau.com\/templates\/patient-discharge-form\/\">patient discharge form<\/a> does the job, and a standard microblading version covers:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Keep the area dry for seven to ten days<\/li>\n\n\n\n<li>No makeup over the brows<\/li>\n\n\n\n<li>No direct sun, sunbeds or swimming<\/li>\n\n\n\n<li>No picking or scratching the scabs<\/li>\n\n\n\n<li>Book the touch-up four to eight weeks out<\/li>\n<\/ul>\n\n\n\n<h3 id=\"h-keep-photo-permission-separate-and-optional\" class=\"wp-block-heading\">Keep photo permission separate and optional<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The template does not include a photography section, so add one if you post before-and-afters. Keep it as its own opt-in, with one box to agree and one to decline.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Say where the photos will appear, whether that is an in-house portfolio, your website, or social media. Add the ones you will use, and leave the rest off.<\/p>\n\n\n\n<h2 id=\"h-california-treats-microblading-as-body-art-not-cosmetology\" class=\"wp-block-heading\">California treats microblading as body art, not cosmetology<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">In California, microblading is not regulated by the Board of Barbering and Cosmetology. It sits on the <a href=\"https:\/\/barbercosmo.ca.gov\/enforcement\/non_jurisdiction.shtml\" target=\"_blank\" rel=\"nofollow noopener\">board&#8217;s own list<\/a> of services outside its jurisdiction. Cosmetic tattooing falls under body art law instead, enforced by county environmental health departments.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That changes who you register with. Body art practitioners register with the local enforcement agency in their county. Registration means proof of age, hepatitis B vaccination evidence, and bloodborne pathogen training.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It changes the form as well. The <a href=\"https:\/\/leginfo.legislature.ca.gov\/faces\/codes_displaySection.xhtml?lawCode=HSC&amp;sectionNum=119303\" target=\"_blank\" rel=\"nofollow noopener\">Safe Body Art Act<\/a> sets a floor for what the consent has to say. A generic download will not meet it on its own.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>A description of the procedure<\/li>\n\n\n\n<li>What the client should expect afterwards, including suggested care<\/li>\n\n\n\n<li>The medical complications that may follow<\/li>\n\n\n\n<li>A statement that body art is permanent<\/li>\n\n\n\n<li>Notice that tattoo inks and pigments are not approved by the Food and Drug Administration<\/li>\n<\/ul>\n\n\n\n<h3 id=\"h-what-other-states-expect\" class=\"wp-block-heading\">What other states expect<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Rules vary. Some states treat microblading as tattooing and license it through a health department. Others fold it into esthetics or permanent cosmetics licensing, and a few barely regulate it at all.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Call the agency that issues your permit before you print anything. Ask what wording is required, and how long you have to keep signed copies.<\/p>\n\n\n\n<h3 id=\"h-how-uk-practices-handle-consent\" class=\"wp-block-heading\">How UK practices handle consent<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">In the UK, microblading is licensed by the local council&#8217;s environmental health team.
