Applying that rule at follow-up is what separates a clean claim from an avoidable denial.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Use S61.441D when all of the following conditions apply:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The wound is in the healing or recovery phase, not under active surgical or procedural management<\/li>\n\n\n\n<li>The patient is presenting for a follow-up check, dressing change, suture removal, or monitoring of the healing wound<\/li>\n\n\n\n<li>The provider performed the initial definitive treatment at a previous encounter, including wound cleaning, foreign body removal, or closure<\/li>\n\n\n\n<li>No new active treatment, surgical intervention, or diagnostic workup is being initiated at this visit<\/li>\n\n\n\n<li>The right hand is the anatomical site, since left hand injuries take the sibling codes<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Do not use S61.441D if the foreign body is still in place and removal is planned or attempted at this visit. That remains an initial encounter, so the code takes the character A. The same test decides the character on a finger wound such as <a href=\"https:\/\/pabau.com\/diagnostic-codes\/icd-10-code-s61238a\/\">S61.238A<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Procedures performed at the follow-up visit are coded separately from the diagnosis.
