A punch cuts around each follicular unit in the donor area, and the grafts come out one by one. Thousands of small incisions then go into the recipient site.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patients rarely hear it described that way. They book something the marketing calls minimally invasive, walk in without a referral, and walk out the same afternoon.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The consent form is where that distance gets closed. Here is what belongs on the page for FUE specifically, what the free template already covers, and the two things it leaves you to add.<\/p>\n\n\n\n<h2 id=\"h-what-an-fue-consent-form-has-to-prove\" class=\"wp-block-heading\">What an FUE consent form has to prove<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A signed form has one job. It shows that the patient made an informed decision before anyone touched their scalp.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For FUE, that decision has four parts:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>The patient knew this was a surgical procedure under local anesthetic.<\/li>\n\n\n\n<li>They were told what the donor area gives up, and that it does not grow back.<\/li>\n\n\n\n<li>They understood that density, growth, and timing are estimates, not promises.<\/li>\n\n\n\n<li>They knew who would be operating on them.<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">The last one is where hair restoration differs from most aesthetic paperwork. Complaints in this field are often less about a bad outcome and more about who did the work.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The other three matter because hair loss carries on after surgery. A patient who was told that at the start reads a thinning crown as biology. A patient who was not reads it as a failed transplant.<\/p>\n\n\n\n<h2 id=\"h-excision-not-extraction-why-the-wording-matters\" class=\"wp-block-heading\">Excision, not extraction: Why the wording matters<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Most consent forms in circulation, including the free template on this page, spell FUE out as follicular unit extraction. That is not what the term means anymore.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In 2018 the International Society of Hair Restoration Surgery, or ISHRS, renamed the procedure <a href=\"https:\/\/ishrs.org\/fue-research-2\/\" target=\"_blank\" rel=\"nofollow noopener\">follicular unit excision<\/a>. An excision is an incision followed by a removal, so the new name describes what the punch does.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The society was blunt about why it made the change. &#8222;Extraction&#8220; made the procedure sound like tweezing, and that impression helped non-physicians sell and perform it.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Two minutes of editing brings your form up to date. Write &#8222;follicular unit excision (FUE)&#8220; at the top, then use FUE for the rest of the page.<\/p>\n\n\n\n<h3 id=\"h-name-who-makes-the-incisions\" class=\"wp-block-heading\">Name who makes the incisions<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The ISHRS <a href=\"https:\/\/ishrs.org\/beware-of-unlicensed-technicians-performing-hair-restoration-surgery\/\" target=\"_blank\" rel=\"nofollow noopener\">warns patients<\/a> that some practices hand the surgical steps to technicians hired by the day. Graft removal, hairline design, and recipient incisions are the steps it names.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Your form is the cheapest way to show you do not work like that. Add three lines to it:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>The name and license number of the surgeon performing the excisions<\/li>\n\n\n\n<li>What assistants do on the day, such as graft sorting or placement<\/li>\n\n\n\n<li>Who the patient contacts if something looks wrong that evening<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Delegation rules differ by state in the US and by registration in the UK. Write down what your team does, then check it against the rule where you practice.<\/p>\n\n\n\n<h2 id=\"h-what-to-put-on-the-form-section-by-section\" class=\"wp-block-heading\">What to put on the form, section by section<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A form that holds up covers eight areas. The template supplies most of them.<\/p>\n\n\n\n<h3 id=\"h-describe-fue-in-the-patient-s-own-language\" class=\"wp-block-heading\">Describe FUE in the patient&#8217;s own language<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The procedure overview sits at the top of the page. The template explains that follicles are removed individually from the back or sides of the scalp, then placed into the thinning area.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Two details are worth adding by hand. Say roughly how many hours the session runs, and confirm the surgery happens under local anesthetic while the patient is awake.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If you quote a graft count at consultation, write it on the form as a range. A patient who was promised 3,000 grafts and received 2,400 has a grievance you could have avoided in one line.<\/p>\n\n\n\n<h3 id=\"h-screen-the-medical-history-before-you-plan-grafts\" class=\"wp-block-heading\">Screen the medical history before you plan grafts<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The template asks the patient to confirm they have disclosed their medications, allergies, and existing conditions. It then runs a yes or no questionnaire covering:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Any current medical conditions<\/li>\n\n\n\n<li>Pregnancy or breastfeeding<\/li>\n\n\n\n<li>Neuromuscular disease<\/li>\n\n\n\n<li>Autoimmune disease<\/li>\n\n\n\n<li>Skin conditions<\/li>\n\n\n\n<li>Known allergies or previous anaphylaxis<\/li>\n\n\n\n<li>Active infection at the treatment site<\/li>\n\n\n\n<li>Antibiotics or other prescription medication<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Three scalp-specific questions are missing, and they change the plan rather than the paperwork. Ask about a keloid history, about blood thinners, and about the type of hair loss being treated.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Alopecia areata is the answer that most often stops a booking. So is a donor area too sparse to supply the coverage the patient wants.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Where a patient lists several prescriptions, collect the detail before the surgery date. A <a href=\"https:\/\/pabau.com\/templates\/hair-transplant-intake\/\">hair transplant intake form<\/a> gives you room to do that properly.<\/p>\n\n\n\n<h3 id=\"h-name-the-risks-patients-raise-afterwards\" class=\"wp-block-heading\">Name the risks patients raise afterwards<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Write the risks the way you would say them across a desk. The template names ten:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Swelling, bruising, and redness at the donor or recipient site<\/li>\n\n\n\n<li>Infection<\/li>\n\n\n\n<li>Folliculitis, meaning inflammation of the hair follicles<\/li>\n\n\n\n<li>Scarring, including keloids in patients prone to them<\/li>\n\n\n\n<li>Itching or temporary numbness<\/li>\n\n\n\n<li>Poor or delayed growth in the transplanted area<\/li>\n\n\n\n<li>Cyst formation<\/li>\n\n\n\n<li>Shock loss, the temporary shedding that follows the procedure<\/li>\n\n\n\n<li>Asymmetry or uneven density<\/li>\n\n\n\n<li>The need for further procedures to reach the result discussed<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Two more belong on an FUE form. The donor area is finite, so every session spends a resource the patient cannot get back.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And FUE leaves scars, whatever the marketing says. They are small round dots rather than a line, but they show on a closely shaved head.<\/p>\n\n\n\n<h3 id=\"h-set-the-timeline-in-months-not-visits\" class=\"wp-block-heading\">Set the timeline in months, not visits<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Shock loss is the reason this section exists. Transplanted hairs shed within weeks, and a patient who was not warned believes the surgery failed.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The template tells patients that results vary and may take six to 12 months to become fully visible. It also states plainly that density, coverage, and growth are not guaranteed.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Add the milestones you use in follow-up. Shedding in the first weeks, early regrowth around three to six months, and final density later still.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Then add one sentence about existing hair. Male pattern loss continues after surgery, so a second session may be needed for reasons that have nothing to do with the first.<\/p>\n\n\n\n<h3 id=\"h-keep-photo-permission-separate-and-optional\" class=\"wp-block-heading\">Keep photo permission separate and optional<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The template bundles photography into the consent statement. The patient authorizes clinical photographs and videos, which are kept with their medical records.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That covers clinical use and nothing else. Marketing use needs its own opt-in, with one box to agree and one to decline.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Say where the images will appear, whether that is your website, social media, or an in-house portfolio. Never make the surgery conditional on a yes.<\/p>\n\n\n\n<h3 id=\"h-put-the-money-terms-in-writing\" class=\"wp-block-heading\">Put the money terms in writing<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The template carries one financial line. Deposits and booking fees are non-refundable unless the practitioner agrees otherwise.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Hair restoration needs more than that, because a second session is common rather than exceptional. State whether the quote covers one session or a plan, and what a further session costs.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Say what happens if graft yield disappoints. A patient who knows the answer in advance rarely argues about it a year later.<\/p>\n\n\n\n<h3 id=\"h-signatures-who-signs-and-when\" class=\"wp-block-heading\">Signatures: Who signs, and when<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The printable template has no signature block at all. That is the single biggest thing to fix before you use it on paper.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Add four lines at the foot of the page:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Patient printed name and date of birth<\/li>\n\n\n\n<li>Patient signature and date<\/li>\n\n\n\n<li>Operating surgeon name, signature, and date<\/li>\n\n\n\n<li>Witness signature, if your insurer asks for one<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Sign it before the day where you can.
