Someone messages the portal about a rash or a blood pressure reading. A provider reads it, thinks about it, and replies. Those minutes are billable, and they usually go unbilled because nobody wrote them down.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">One distinction matters before anything else. A single 7-day inquiry cannot be split into two claims, so only one code from the family applies per episode.<\/p>\n\n\n\n<h2 id=\"h-who-can-bill-the-code-and-who-can-t\" class=\"wp-block-heading\">Who can bill the code, and who can&#8217;t<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Only physicians and QHPs enrolled in Medicare under their own National Provider Identifier (NPI) may report <strong>CPT code 99421<\/strong> independently. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That limit bites hardest in mixed teams, which is a daily reality for <a href=\"https:\/\/pabau.com\/industry\/gp-clinic-software\/\">primary care practices<\/a> handling heavy portal traffic.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Physicians (MD\/DO):<\/strong> May bill 99421 independently.<\/li>\n\n\n\n<li><strong>Nurse practitioners (NPs) and physician assistants (PAs):<\/strong> May bill under their own NPI as QHPs, subject to their state scope of practice.<\/li>\n\n\n\n<li><strong>Clinical nurse specialists (CNS) and certified nurse midwives (CNM):<\/strong> Eligible as QHPs where state law permits.<\/li>\n\n\n\n<li><strong>Registered nurses, medical assistants, and other clinical staff:<\/strong> Cannot bill 99421 independently. Their time may count toward the cumulative total under incident-to supervision, but the claim goes out under the supervising physician or QHP&#8217;s NPI.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Incident-to arrangements complicate all of this. When the supervision picture is unclear, bill under the supervising physician&#8217;s NPI rather than risk a compliance finding. Read it next to your own payer contracts.<\/p>\n\n\n\n<h2 id=\"h-the-patient-has-to-start-the-conversation\" class=\"wp-block-heading\">The patient has to start the conversation<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Patient initiation is a hard requirement, not a preference. The 7-day clock starts only when the patient, or an authorized representative such as a parent or guardian, sends the first message. A provider-prompted exchange does not qualify at all.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Here is where the line falls:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Qualifies:<\/strong> A patient messages the portal about a medication side effect, a refill, or a new symptom.<\/li>\n\n\n\n<li><strong>Qualifies:<\/strong> A parent messages the portal on behalf of their minor child.<\/li>\n\n\n\n<li><strong>Does not qualify:<\/strong> The provider sends a follow-up asking how the patient responded to a treatment change.<\/li>\n\n\n\n<li><strong>Does not qualify:<\/strong> Staff send an automated notification asking the patient to check in about a chronic condition.<\/li>\n\n\n\n<li><strong>Does not qualify:<\/strong> The message concerns a problem already addressed at an office visit inside the same 7-day period.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Check that your <a href=\"https:\/\/pabau.com\/blog\/benefits-of-patient-portals\/\">patient portal software<\/a> timestamps the patient&#8217;s opening message separately from automated notifications. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That log becomes your initiation record at audit, and solid <a href=\"https:\/\/pabau.com\/blog\/patient-care-management\/\">patient care management<\/a> tooling keeps it intact without extra admin work.<\/p>\n\n\n\n<h2 id=\"h-documentation-that-survives-an-audit\" class=\"wp-block-heading\">Documentation that survives an audit<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Documentation is where most 99421 denials start. The record has to support both the time threshold and the clinical substance of the exchange. A note reading &#8222;responded to patient portal message&#8220; will not hold up under review.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Your record needs all of this:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Date and time of the patient&#8217;s first message,<\/strong> which sets the start of the 7-day window.<\/li>\n\n\n\n<li><strong>Cumulative time spent by the billing provider<\/strong> reviewing, researching, and replying. Staff time only counts where incident-to rules genuinely apply.<\/li>\n\n\n\n<li><strong>Content of the clinical exchange:<\/strong> the patient&#8217;s question, the provider&#8217;s reasoning, and any advice, orders, or referrals issued.<\/li>\n\n\n\n<li><strong>Confirmation that the patient is established,<\/strong> because new patients are not eligible for this code family.<\/li>\n\n\n\n<li><strong>Date the 7-day period closed,<\/strong> plus the cumulative total that decided the code tier.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Undercoding is the quieter problem. Log time message by message and you lose the running total.
